Job Title: Health Information Clerk

Compensation (Based on experience and qualifications): Starting at $16.75

Location: Annex 9119 S. Exchange Avenue, Chicago IL 60617

Schedule: Monday - Friday 8:15am - 5:00pm

We are seeking a Health Information Clerk to join our team. As a Health Information Clerk you will collect, maintain, and make available to authorized users accurate and complete patient health information in a timely manner.

Requirements:

  • Must be able to use a computer and be familiar with its functionality.
  • Serves as a liaison daily to physicians regarding issues related to incomplete records and documentation, to ensure adherence to JCAHO standards and compliance requirements.
  • Produces and distributes delinquency notifications immediately to providers for each occurrence of deficient information.
  • Receives and processes requests for records within 30 days of receipt in accordance with Illinois' release of information policies and procedures and any applicable legal regulations.
  • Document via quick note the reason for accessing the patient's EHR.
  • Analyzes the content of all returned medical records for missing progress notes, diagnostic reports, lab results, and other medical documents. All completed documentation must have appropriate signatures appended and follow guidelines established by CFHC, JCAHO and all other applicable policies and/or regulations.
  • Prior to delivery, correctly organizes documents in each patient's record according to the chart order policy.
  • Keeps the requester of any record/report that was not found, notified of efforts to locate.
  • Performs other work as needed/assigned.

Education and Experience

  • High school diploma and 1 year of previous experience working in a health information department or business office environment.
  • Proficiency with personal computer systems and software commonly found in an office environment is required.
  • Must possess good written and verbal communications skills and be able to manage confidential information.
