Shift:
8:00 AM - 4:30 PM

Experience

  • 12 months of role experience is required with some in the last 12 months.
  • Preferred 24 months of Case Management experience.
  • Experience with Epic is preferred from any number of months.
  • ADN completion required

Requirements

  • Candidates must have a Michigan license (required for submission).
  • Flu vaccination required after submission. Religious and medical declinations accepted.

Integrated Case Manager
Required Skills/Experience

  • Minimum 5 years of Acute Inpatient Case Management experience (this does NOT include telephonic, utilization management, or therapy experience).
  • Acute discharge planning experience, Recent (within the last two years) acute inpatient case management experience.
  • Highly experienced candidates with acute inpatient case management and discharge planning experience.

Required Credentials

  • Nursing degree or MSW.
  • Current valid MI RN licensure or LMSW with current valid MI licensure.

Shift & Scheduling:
Rotating weekends.

7 days maximum time off

1 reference from any number of months (Manager/Supervisor reference type) - required for submission

1 reference from any number of months (Any reference type) - required for submission
