Position OverviewWe are seeking a Nurse Practitioner or Physician Assistant to provide full-spectrum outpatient primary care to patients across all ages. This role emphasizes continuity of care, patient education, and improving health outcomes within our community.
Key Responsibilities- Conduct comprehensive physical exams and review diagnostic results
- Diagnose and treat acute illnesses (e.g., infections, minor injuries)
- Manage chronic conditions including diabetes, hypertension, heart disease, and obesity
- Develop and implement individualized treatment plans
- Provide patient-centered care addressing physical, emotional, and social needs
- Collaborate with physicians, nurses, and specialists for coordinated care
- Educate patients and families on conditions, treatment options, and wellness strategies
- Coordinate referrals, follow-ups, and hospital admissions when necessary
- Maintain accurate, timely documentation in the EMR
- Lead wellness screenings and preventive health initiatives
- Maintain current clinical knowledge through continuing education
- Active Nurse Practitioner or Physician Assistant license in Arizona (or eligibility in progress)
- Graduate of an accredited U.S.-based NP or PA program (Puerto Rico programs not accepted)
- Current or eligible DEA registration
- BLS certification required; ACLS preferred
- Minimum 1 year of primary care experience preferred

Requirements
Valid Nurse Practitioner or Physician Assistant license in the State of Arizona (or in process)
Completion of a NP/PA Program in USA (Not Puerto Rico)
