Administrative Specialist (Care Management Department)

Position Responsibilities:

  • Initiates telephone contacts with post-acute and community resources at the request of Care Manager/Social Worker to support patient discharge arrangements. Determines availability of services specified.
  • Collects, copies and transmits pertinent clerical and patient demographic information required to complete arrangements for post acute care and/or placement as directed. 
  • Confirms post-acute arrangements with providers, communicating date/time and method of transfer. Documents communicated information according to department policy and procedures. 
  • Arranges post-acute transportation, DME and community services for patient in acute care setting as directed. 
  • Communicates post-acute service availability, barriers to timely discharge/transfer and issues requiring Social Work/Case manager follow-up to appropriate staff members.
  • Maintains regular communication with Social Worker, Case Managers and Case Management leadership. 
  • Maintains an adequate inventory of office supplies based on usage patterns; prepares all purchase requisitions and disbursements. 
  • Provides secretarial and clerical support including faxing, filling and typing. 
  • Updates forms and reference files to ensure that they are current all times as requested. 
  • Screens telephone calls and identifies urgency of issues initiates appropriate referrals and pages promptly. 
  • Performs related clerical and record management responsibilities as required. 
  • Assumes primary responsibility for the delivery and distribution of mail in the department. 
  • Promotes service orientation in the performance of position duties and responsibilities and in interactions with patients, hospital staff and visitors.

Qualifications:

  • High School Diploma or GED Preferred.
  • Must have extensive knowledge of computers, calculators and general office equipment.
  • At least 2-3 years of experience in clerical related duties.
