Overview

FLSA: Non-Exempt: This is a position in the assisted livings of the Long-Term Care Campuses of La Crosse County. Employees are responsible for the overall coordination of their assigned household. Employees are appointed by and responsible to the Assisted Living Supervisor or designee.

Location: The Evergreens at Hillview

Schedule: Monday through Friday, 8 a.m. to 4:30 p.m. Participant in a rotational on-call for nights, weekends, and holidays. 

Pay Scale: $ $25.66/hour; Offered wage depends on experience

Essential Job Functions

Coordinates individual tenant or resident schedules (i.e. work sites, appointments, and community events).

Prepares, plans, facilitates and delegates household and individual activities. Provides ongoing programs, maintains supplies and materials for various programs.

Completes care planning or individual service plans, as assigned and collaborates with Registered Nurse. Develops and reviews appropriate documentation.

Facilitates necessary paperwork with families (Risk Agreement, Service/Care Agreement, reviews handbook, policies and procedures), gives tours of assisted living, and is available for questions. Educates tenant or resident families about services and established agreements.

Trains, supervises and evaluates staff. In conjunction with the assigned scheduler, prepares work schedules according to census, assigns staff to shifts, and ensures sufficient staff on each shift and area. Confirm staff records are current and complete.

Adheres to federal and state regulations, assists in development of policies, and in-service education programs for staff according to specific licensures.

Coordinates programs, in-services, and handles public relations concerns.

Makes daily rounds to monitor quality of household services. Assure appropriate use of staff resources, oversee appropriate use of supplies, food and equipment, and determine need for additional or replacement equipment.

Monitors and participates in the appropriate use of budget and keeps supervisor informed of budgetary status.

Attends meetings, service planning conferences, and in-service educational programs.

Communicates effectively with responsible parties related to tenant or resident needs.

Involves Assisted Living Supervisor to include Corporation Counsel with legal issues or concerns/liabilities. Advises any change made to contracts, agreements, or handbook.

Works well with all generations and diverse populations. Works through difficult situations that may arise and uses conflict resolution. Provide staff coverage in event of a staffing shortage.

Communicates with appropriate assisted living staff and outside agencies for referrals, admissions, discharge between facilities, financial, psychiatric, restorative and social problems of which the facility cannot meet.

Develops and publishes ongoing assisted living communications and calendar of events. Promotes marketing events. Conducts quality tenant satisfaction surveys.

Provides transportation to and from recreation program areas. Drives facility vehicles: responsible for advising supervisor if valid driver's license and good driving record is not maintained.

Supports all aspects of person-centered care. Respects and acknowledges individuality and uniqueness.

Maintains resident rights and confidentiality of resident information. Immediately report all suspected cases of resident abuse or neglect to the supervisor or Administrator.

In this public service position, employee is required to be courteous, cooperative and respectful at all times, with the public and clients. Also establishes and maintains a courteous, cooperative and respectful working relationship with other employees, supervisors and public officials.

Works safely and follows safety practices and standards of the facility; responsible for reporting or correcting any existing or potential safety or accident hazard.

Reports to work on time and when scheduled.

Physical demands: Sits, stands, bends, lifts and moves around facility intermittently during working hours. Lifts and transports tenants or residents; pushes and pulls wheelchairs, transfers tenants or residents from wheelchair to wheelchair or to regular chair. Ability to lift 60 pounds. Reads, uses judgement, writes, uses near vision and handles various objects. Bends and stoops to a squat within six inches of the floor for putting on tenant or resident's shoes, for example.

Training, Experience and Other Requirements

Associates degree in a related health or human services field and experience as a Resident Care Assistant in a Residential Care Apartment Complex (RCAC) or Community Based Residential Facility (CBRF) are preferred. May consider other relevant combinations of education and experience that provide equivalent knowledge, skills, and abilities.

A valid Wisconsin Certified Nursing Assistant (CNA) registration and Community Based Residential Facility (CBRF) registry validation are preferred. CBRF staff must be listed on the Wisconsin CBRF Employee Registry as having completed the four core DHS 83 courses—Standard Precautions and Medication Administration (prior to related duties), and Fire Safety and First Aid & Choking (within 90 days of hire).

Employee driving County vehicles must have and maintain a valid driver's license and a good driving record.

Must be flexible and available to work some evenings, weekends, and holidays in an on-call rotation.

Must pass criminal record check as required by the Wisconsin Caregivers Law.
