Purpose:

The Senior Case Manager will work with the Contra Costa school district and schools to provide interventions including basic needs support to ORR-eligible youth and families, fostering family engagement and empowerment, supporting youth engagement and development, supporting school enrollment, and supporting capacity-building through participation in communities of practice.

This is a two-year limited School Impact Grant position, ending in July 2026, with continued employment contingent upon future funding.

Responsibilities:

  • Responsible for the day-to-day operations of the program.
  • Responsible for collaborating and coordinating services with staff at multiple school districts and schools where Ukrainian ORR-eligible youth are enrolled or planning to enroll in Alameda, Contra Costa and San Francisco counties to ensure student and family needs are met.
  • Coordinate and facilitate family/school orientations, registration, resources, and activities for new Ukrainian ORR-eligible arrivals.
  • Responsible for entering accurate client information, case notes in a timely manner, and all other required forms and documents.
  • Responsible for ensuring that case files are in accordance with specifics of the School Impact Grant standards and procedures.
  • Responsible for preparing monthly, quarterly and annual reports.
  • Ensure family understanding of school attendance requirements. Conduct home visits as needed.
  • Assess the needs of students and/or family members, identify risk factors, and create a case plan that outlines goals and objectives.
  • Work with families to develop appropriate plans for their children, such as developing a parenting plan or creating a career plan. Monitor progress on goals and objectives, communicating with parties involved in the case, and adjusting as needed.
  • Maintain ongoing, effective communication with caregivers, schools, and the community; develop and support family outreach programs
  • Facilitate increased School Impact Grant involvement in school events and Parent/Teacher conferences (Back to School events, ELAC meetings, parent-teacher conferences, Open House, site specific activities).
  • Support students' participation in summer and after school programming.
  • Provide High School students with university preparation assistance.
  • Support families with translation of transcripts and other documentation.
  • Review report cards from the school or other professionals involved in the case, such as teachers and tutors, to ensure understanding and that progress is being made.
  • Coordinate tutoring sessions to provide school assistance to improve GPA results and help students to achieve their goals.
  • Network and develop partnerships within the local community to educate providers on children's needs and to reinforce/increase partnerships in relevant areas.
  • Participate in local and regional meetings and training that pertain to service provision for the target population.
  • Travel regularly throughout the service delivery area (home visits, agency visits, service provider visits etc.).
  • Perform other related duties and special projects as requested.

Qualifications:

Education and Experience:

  • Bachelor's Degree or equivalent work experience in social work, education, psychology, sociology, or other behavioral science in lieu of degree.
  • A minimum of 4 years' experience in case management, including working with children and youth, previous work experience and interest working with foreign born populations.

Skills:

  • Must have superior written and verbal communication skills. Language fluency in written and spoken English in addition to Ukrainian or Russian is required.
  • Knowledge and experience in working with Microsoft Word, Excel, Power Point and database software is essential.
  • Strong professional ethics and sensitivity in dealing with confidential matters.
  • Excellent interpersonal skills, the ability to work independently and as part of a in a multi-cultural team.
  • Possession of a valid California state driver's license and access to a reliable vehicle.
