We are seeking a dynamic and dedicated Home Care Registered Nurse Supervisor to lead and oversee our home health nursing team. Responsibilities

  • Supervise and support registered nurses and home health aides to ensure delivery of high-quality patient care.
  • Develop and implement individualized care plans in collaboration with healthcare providers and family members.
  • Monitor clinical practices to ensure compliance with state and federal healthcare regulations, accreditation standards, and organizational policies.
  • Conduct regular performance evaluations, provide coaching, and facilitate ongoing training for nursing staff.
  • Coordinate scheduling, assignments, and resource allocation to optimize service delivery across the home care team.
  • Oversee patient assessments, documentation accuracy, and timely reporting of clinical outcomes.
  • Act as a liaison between patients, families, physicians, and community resources to promote coordinated care.
  • Investigate incidents or complaints promptly, ensuring appropriate corrective actions are taken to maintain safety standards.
  • Keep abreast of industry best practices and participate in continuous quality improvement initiatives.

Skills

  • Valid Registered Nurse (RN) license with current state licensure.
  • Proven leadership experience in home health or community-based nursing settings.
  • Strong knowledge of home care regulations, Medicare/Medicaid guidelines, and clinical best practices.
  • Excellent communication skills for effective collaboration with multidisciplinary teams, patients, and families.
  • Ability to organize workload efficiently while managing multiple priorities under pressure.
  • Proficiency in electronic health records (EHR) systems and documentation protocols.
  • Compassionate approach with a commitment to patient-centered care.
