Overview

We are seeking a dedicated and energetic Master Social Worker (Home Health) to join our dynamic healthcare team. Duties

• Conduct thorough patient assessments to evaluate social, emotional, and environmental needs using diagnostic evaluation tools and clinical judgment.
• Develop individualized care plans that incorporate behavioral health strategies, mental health counseling, and crisis intervention techniques tailored to each patient's circumstances.
• Provide individual and group therapy sessions utilizing evidence-based practices such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy to support mental health recovery.
• Offer specialized counseling for child and family dynamics, including working with children with developmental disabilities or autism spectrum disorders.
• Coordinate with healthcare providers to ensure compliance with HIPAA regulations while managing medical documentation, medical records, CPT coding, ICD-9/10 coding, and other clinical documentation requirements.
• Facilitate intake procedures for new patients and manage ongoing case management activities to promote optimal health outcomes in home and community settings.
• Assist patients in navigating post-acute care options such as assisted living or nursing homes while providing hospice care support when necessary.
• Collect data related to patient progress and treatment efficacy for clinical research initiatives or quality improvement projects.
• Engage in crisis management and intervention for patients experiencing PTSD, dementia-related behaviors like Alzheimer's care challenges, or other urgent mental health needs.
• Collaborate with multidisciplinary teams to deliver comprehensive outpatient or inpatient services across various settings including clinics, hospitals, nursing homes, or assisted living facilities.
• Maintain accurate medical documentation and records using electronic medical record (EMR) systems while adhering strictly to HIPAA standards for patient confidentiality.

Requirements

• Master's degree in Social Work (MSW) from an accredited institution; licensure as a Licensed Clinical Social Worker (LCSW) or equivalent required.
• Proven experience in home health care settings with a strong background in behavioral health, mental health counseling, or clinical social work.
• Demonstrated expertise in child & family counseling, addiction counseling, dementia care including Alzheimer's disease management, PTSD care, and working with individuals with disabilities or developmental delays.
• Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and medical documentation practices essential for accurate record keeping.
• Experience working within EMR systems and conducting data collection for research or quality assurance purposes preferred.
• Strong skills in crisis intervention techniques coupled with the ability to perform diagnostic evaluations effectively.
• Knowledge of HIPAA regulations ensuring patient privacy and confidentiality at all times.
• Excellent communication skills for engaging patients across diverse populations including pediatrics, geriatrics, and special education needs.
