PURPOSE:

The Team Leader ("TL") is responsible to lead, direct work, train & develop, and manage Vocational Counselors (VCs).

Ø TEAM LEADERS UNDERSTAND THEIR TEAMMATES REPRESENT THE TRUE VALUE OF EAG.

The PC ensures VCs have the proper tools, resources and skill sets to perform their person-centered roles effectively.

This position will be on-call, i.e. available for emergency coverage, as scheduled by the Program Coordinator.

We appreciate that Vocational Counselors are in the field every day performing their work, and may meet prospective clients. These individuals should be brought to the attention of the Program Coordinator for referral to the Outreach & Client Recruitment Coordinator.

KEY RESPONSIBILITIES:

· Provide SUPERVISION, direct work, train & develop, and manage VCs. Ensure VCs have the proper tools and skill sets to provide person-centered services for positive outcomes

o LEAD VCs. Positively motivate and encourage staff to strive for excellence in service

o Provide COACHING and honest one-on-one feedback for continuous improvement, job performance & motivation

o QUICKLY bring VC feedback, concerns, and opportunities for improvement to the attention of the Program Coordinator for action, implementation, or resolution

o Lead Team meetings regularly and frequently, being PREPARED and MOTIVATIONAL

o Conduct weekly documentation audits of VC notes, etc. to ensure compliance and completeness

o Create employee schedules and ensure billable hours are met, and submitted/recorded correctly

o Maintain program records and prepares reports as required and requested

o Provide instruction and documentation on the implementation of outcomes in their Individual Service Plan (ISP)

o Implement Individual Service Plans (ISP)

o Participate in obtaining and maintaining CARF accreditation

o Ensures accurate and adequate billing is completed in a timely manner

o Perform other duties as assigned

· Perform all duties of the Vocational Counselor/ Job Coach

(Vocational Counselor/ Job Coach Responsibilities)

  • Provide continuous job support and fading in the community

  • Cultivate and maintain appropriate networking sources with the community

  • Maintain open communication with the individuals' employment sites

  • Attend and participate in all relevant agency events, workshops, training and meetings;

  • Prepare written reports in the appropriate format for the referral source on a monthly, quarterly, or as-needed basis

  • Assist clients with application process and interviews, and coordinate arrangements for job placement

  • Monitor and evaluate client work performance, collect skill acquisition and production data, obtain feedback from client, employer, and coworkers, and provide additional training or intervention as needed

  • Assess client skills and abilities and requirements of specific employment situations for the purpose of job/client matching

  • Provide current information on clients via the vocational profile form

  • Provide on-going assessment and follow-along services as needed and authorized

  • Prepare and update written documentation for clients, employers, and counselors in a timely manner

  • Provide job site consultations and backup support as needed

  • Perform other duties as assigned

Requirements:

· High school diploma or GED; (Associate's degree or equivalent work history preferred).

· One year of experience working with individuals with disabilities (one year of supervisory experience preferred).

· Ability to communicate clearly and effectively with clients and staff verbally and in writing.

· Meet the basic personnel requirements for Criminal History, CPS and DMV record checks, and HHS-OIG.

· Ability to obtain and maintain CESP certification within 2 years

· Obtain an evaluation indicating the absence of TB in a communicable form.

· Must obtain and maintain current certification in CPR and First Aid

· Ability to interact with families in a positive, supportive, and professional manner.

· Availability to work various hours and shifts, both day and night;

· Must have reliable/available transportation, maintain a valid/acceptable driving record/license and be able to produce proof of vehicle insurance.

· Maintain an active mobile telephone number for required contacts about assignments.

· Position may require lifting, running, and kneeling, and ability to lift up to 50 pounds.
