The Community Licensed Vocational Nurse (LVN) plays a vital role in delivering high-quality, patient-centered care within Pacific Health Group. Under the direct supervision of a Registered Nurse (RN), the LVN provides nursing care to individuals and families in their homes, community clinics, and other designated outreach locations. This role focuses on promoting health, preventing illness, and managing chronic conditions through education, assessment, and direct care, while adhering to all established policies, procedures, and regulatory requirements.

Key Responsibilities:

Direct Patient Care:

Complete medication reconciliation within the scope of LVN practice.

Perform skin assessments, vital sign monitoring, and basic nursing procedures.

Assess the patient's ability to perform activities of daily living.

Collect and document patient data, including health history, current symptoms, and responses to interventions.

Reinforce patient education as delegated by the RN, ensuring patient safety and comfort.

Check the status of appropriate specialists' referrals

Ensure members have medical devices and supplies: glucometer, BP cuff, scales.

Ensure members have medical logs: Vitals, Blood sugar, Exercise.

Alert RN of patients' hospitalizations and assist with care plan updates.

Support smooth transitions of care, including hospital discharges and movement between care settings.

Facilitate PCP visits within 14 days of discharge: admissions & ER visits.

Health Education and Promotion:

Empowers patients and their families on aspects like disease management, medication adherence, healthy lifestyle choices, and preventive care.

Provide instruction on self-care techniques and the use of medical equipment.

Reinforce health teachings provided by the RN.

Assessment and Documentation:

Observe and report changes in patient condition to the supervising RN promptly.

Accurately and thoroughly document all nursing interventions, patient responses, and communications in the electronic health record (EHR).

Contribute to the development and revision of patient care plans under the direction of the RN.

Collaboration and Communication:

Work collaboratively with the interdisciplinary healthcare team, including RNs, lead care managers, community health workers, Therapists, physicians, social workers, and other allied health professionals.

Communicate effectively with patients, families, and caregivers, demonstrating empathy and respect.

Participate in team meetings and case conferences as required.

Community Outreach:

Assist in community health screenings, home visits, and other public health initiatives.

Provide nursing support at various community locations, which include home visits and assessments.

Compliance and Professionalism:

Adhere to all federal, state, and local nursing regulations and professional standards of practice.

Maintain patient confidentiality and privacy in accordance with HIPAA regulations.

Participate in continuing education and professional development activities to maintain licensure and enhance nursing skills.

Demonstrate professionalism, integrity, and ethical conduct at all times.Collaboration with Lead Care Managers (LCMs): LVN will conduct multiple on-site visits each month in patients' homes, shelters, or community centers accompanied by lead care managers (LCMs) assigned to cases. LVN will use these visits to establish trust, gather first-hand insights, and address concerns right away. Close collaboration with LMCs is crucial for the success of this role. Example: While visiting a member recovering at home, you might discover that they lack mobility aids—prompting you to collaborate with the assigned lead care manager to arrange for durable medical equipment and coordinate in-home physical therapy.

Qualifications

Education: Graduate of an accredited Licensed Vocational Nurse (LVN) program.

Licensure: Current, unencumbered LVN license in the State of California

Experience: Minimum of 2 years of clinical nursing experience, preferably in: community health, home health, health centers, acute or post-acute care, public health programs, behavioral health, or SUD treatment programs.

Experience working with Medi-Cal populations, including individuals who are: Homeless or at risk of homelessness, justice-involved, living with SMI/SUD, facing multiple chronic health conditions.

Culturally-sensitive and trauma-informed approach to patient care: Understands how trauma, racism, poverty, and systemic barriers affect health outcomes.

Bilingual proficiency (especially in Spanish, Mandarin, Vietnamese, or Tagalog) is a plus.

Residency: Must reside in Santa Clara County

Successful completion of a pre-screen assessment required

Certifications: Current Basic Life Support (BLS) certification required

Skills:

Excellent clinical nursing skills appropriate for the LVN scope of practice.

Strong interpersonal and communication skills, with the ability to build rapport with diverse patient populations.

Ability to work independently and as part of a team.

Strong organizational and time management skills.

Proficiency in electronic health record (EHR) systems.

Ability to adapt to changing environments and prioritize tasks effectively.

Compassionate and patient-centered approach to care.

Other: Possess a valid California Driver's License (Class C minimum), maintain a personal, operable vehicle for daily business use, and carry current liability insurance that meets California's minimum legal requirements. All selected candidates will be required to pass a Motor Vehicle Report (MVR) background check prior to employment.

Physical Demands:

Ability to stand, walk, bend, lift, and carry objects up to 25 pounds.

Ability to perform tasks requiring manual dexterity and visual acuity.

Ability to work in various community settings, including patient homes, which may have varying environmental conditions.

Work Environment:

Primarily community-based, including patient homes, community clinics, and outreach locations.

May involve exposure to various environmental conditions and potential communicable diseases.

Regular travel within the community is required.

LVN will conduct frequent home visits and assessments with lead care managers assigned to patients.
