Case Manager Supervisor

Our organization is a startup in behavioral healthcare in New Jersey. We support people with mental illness and Substance Abuse as they achieve greater independence and life satisfaction through a wide range of services.

JOB SUMMARY:

Case Manager Supervisor – Provides guidance on best practices in care coordination and oversight to Sr. Case Managers/Case Managers. The successful candidate will assist with daily living activities and provide behavioral support, medication management and crisis intervention in a group home setting.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Supervises both Sr. Case Managers/Case Managers with coaching, training, and development ensuring that cases are distributed appropriately and case management services are delivered in accordance with contractual agreements and regulatory standards.
  • Coordinates Sr. Case Manager/Case Manager scheduling to ensure sufficient coverage in all group homes and operational needs.
  • Assist with the implementation and compliance of new policy and procedures and compliance of existing policy and procedures to include the monitoring of patient medication.
  • Ensure that all Case Managers maintain current and accurate confidential case records, which includes intake, assessment, follow-up evaluations, progress reports, consultations and recommendations to be "Audit Ready" at all times.
  • Provides positive behavioral supports on a daily basis and intervenes appropriately to manage and avert crisis situations.
  • Must be able to travel to houses within service areas and cover down as shift coverage for employee call outs.
  • Collects, Records, and Provides data for input into assessments, conferences and treatment planning
  • May conduct employment interviews in accordance with company procedures.
  • Responsible for ensuring a safe, therapeutic environment and adhering to all safety standards, Federal and State regulations, and corporate policies

EDUCATION, KNOWLEDGE, SKILL & ABILITY:

  • Bachelor's Degree or higher and/or seven (7+) year of work or life experience.
  • Minimum three years' experience as a case management supervisor or management required.
  • Manager/supervisory experience required.
  • Recent personnel management experience required.
  • Excellent interpersonal and communications skill.
  • Ability to demonstrate respect for individuals with I/DD and their families.
  • Ability to lift 25 pounds.

Certifications :

  • Maintains current CPR, AED and First Aid Certification as well as Nurse Safe Training and Fire Training; If the applicant doesn't have all certs, they must obtain certs within 3 months of employment.
  • Relevant certifications: i.e. (CADC, QMAP, CCAR, KEMP, Peer Coach, etc.)

  • Full-time and Part Time Shifts available (7 days a Week, weekdays 4pm - 7am, weekend shifts between 8am- 8pm )
