Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with expertise spanning various clinical settings including long-term care, assisted living, and acute care environments. As an LPN, you will play a vital role in delivering high-quality nursing services, supporting patient recovery, and ensuring compliance with healthcare standards. Your proactive approach and attention to detail will contribute significantly to the well-being of our diverse patient population.
Duties
- Provide direct patient care by monitoring vital signs, administering medications, and performing routine assessments in accordance with physician orders and established protocols.
- Assist with medical procedures such as IV infusion, catheterization, phlebotomy, and sterile dressing changes while maintaining aseptic technique.
- Support patient mobility and comfort through proper positioning, tube feeding management, and use of assistive devices like Hoyer lifts.
- Document patient information accurately using Electronic Health Records (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams including physicians, therapists, and case managers to develop and implement individualized care plans.
- Educate patients and their families on health management strategies, including medication administration, nutrition, and disease-specific care such as dementia or Alzheimer's.
- Participate in discharge planning, post-acute care coordination, and community outreach efforts to promote continuity of care.
- Maintain infection control standards through proper hand hygiene, equipment sterilization, and adherence to safety protocols across all clinical activities.
Experience
- Valid licensure as a Licensed Practical Nurse (LPN) in accordance with state or regional requirements.
- Prior experience working in diverse healthcare settings such as hospitals (including ICU or trauma centers), nursing homes, hospice care, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation.
- Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures, and familiarity with medical terminology relevant to acute and chronic conditions.
- Experience managing complex patient needs including pain management, infectious disease care, behavioral health issues such as dementia or memory care patients.
- Skilled in performing physical examinations, specimen collection/processing, and basic life support (BLS).
- Ability to work effectively with special populations including pediatrics, neonatal ventilator patients, individuals with developmental disabilities or those requiring assisted living services.
