About this role:

As an Advanced Practice Provider in Palliative Medicine, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will make home visits to help patients set goals of care, manage symptoms and medications, and coordinate care.

• This position includes a $10,000 sign-on bonus and $10,000 in relocation assistance, when applicable

• Your Schedule

• Monday – Friday, 8am to 5 pm

• Hybrid remote and home visits

What you'll bring:

• APRN or PA-C degree from an accredited program

• 1-2 years' experience as an APP in specialty care, palliative care strongly preferred

• NO New Grads, Please

• Active, unobstructed Colorado medical license, or the ability to obtain one
• Full prescriptive authority and an active DEA, or the ability to obtain one
• Ability to successfully complete Intermountain Health's credentialing process
