Nursing Facility Transition

Position Overview
The NFT Specialist provides direct support to Medicaid recipients living in nursing facilities who want to transition back into the community. This role focuses on person-centered planning, resource coordination, and ongoing follow-along support to promote independent living.
Key Responsibilities
- Provide direct services to nursing facility residents interested in community-based living.
- Conduct assessments and develop person-centered transition and independent living plans.
- Coordinate all aspects of the transition plan and make referrals as needed.
- Deliver information and referral on community resources.
- Complete outreach to nursing facilities and residents regarding available services.
- Maintain accurate documentation within 24 hours and manage consumer files.
- Submit reports and meet assigned part-time benchmarks.
- Participate in state and community meetings related to Long-Term Care/NFT as requested.
- Conduct weekly follow-along calls and monthly in-person visits per NFT guidelines.
- Travel throughout the assigned service area.
Required Competencies
- Knowledge of HIPAA and Mandated Reporter responsibilities.
- Understanding of person-centered processes.
- Knowledge of developmental and mental health disabilities.
- Strong communication, organization, and time management skills.
- Ability to work independently and maintain confidentiality.
- Respect for consumer choice and comfort with outreach and public speaking.
- Basic computer proficiency and ability to track timelines and due dates.
Qualifications
Candidates must meet ONE of the following:
- Licensed Registered Nurse in Michigan, OR
- Licensed Social Worker in Michigan, OR
- Bachelor's degree in a health or human services field or Community Health Worker certification and at least 3 years of relevant experience.
Additional Requirements:
- Commitment to the Independent Living Philosophy.
- At least 3 years of experience working with community resources.
- Ability to travel to nursing facilities, consumer homes, and community sites, including locations with stairs.
Work Expectations
- Maintain a manageable caseload (scaled to part-time).
- Cover assigned nursing facilities monthly when applicable.
- Complete all documentation within 24 hours.
- Attend scheduled agency meetings and trainings.
- Work 20 hours per week with occasional flexibility outside standard business hours.
Work Environment & Travel
Work is performed in office settings, consumer homes, nursing facilities, and community sites. Regular travel within the service area is required; occasional out-of-area travel may occur.
Other Duties
Responsibilities may change at any time based on program or agency needs.
Pay: $20.00 per hour
Expected hours: 20.0 per week
