Overview
Houston Memory Clinic is seeking a compassionate and experienced Nurse Practitioner (NP) to join our dedicated team. This position focuses on conducting Medicare Annual Wellness Visits (AWVs) for dementia patients residing in nursing homes and providing telehealth services as needed. The ideal candidate will have experience working with older adults, particularly those with cognitive impairments, and be skilled in delivering patient-centered care in collaboration with caregivers, family members, and interdisciplinary care teams.
Key Responsibilities
- Conduct Medicare Annual Wellness Visits:
- Perform comprehensive health risk assessments, cognitive evaluations (e.g., Mini-Cog, MoCA, SLUMS), and functional assessments for nursing home residents.
- Review and update medical histories, preventive care, and vaccination status.
- Identify cognitive and behavioral concerns while supporting diagnoses related to dementia and aging.
- Develop individualized care plans in collaboration with patients, caregivers, and facility staff.
- Telehealth Services:
- Provide virtual evaluations and follow-up wellness visits for nursing home residents when on-site care is not feasible.
- Educate caregivers on telehealth platforms, ensuring seamless virtual care delivery.
- Dementia-Focused Interventions:
- Address memory, mood, and behavioral challenges with tailored support strategies.
- Coordinate referrals to specialists (e.g., neurology, speech therapy, physical therapy) as needed.
- Collaborate with nursing facility staff to implement and monitor cognitive rehabilitation plans.
- Documentation and Compliance:
- Accurately document patient visits in adherence to Medicare standards and legal requirements.
- Ensure timely submission of clinical notes, care plans, and follow-up recommendations.
- Maintain up-to-date knowledge of Medicare policies and guidelines associated with Annual Wellness Visits and dementia care.
Qualifications
- Current and unrestricted Nurse Practitioner license in the state of Texas.
- Board Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).
- Minimum of 2+ years experience in dementia care, geriatric nursing, or a related field.
- Experience conducting Medicare Annual Wellness Visits with knowledge of CMS guidelines.
- Familiarity with nursing home and long-term care settings.
- Proficiency in telehealth technology and virtual care delivery platforms.
- Strong knowledge of dementia screening tools (e.g., Mini-Cog, MoCA, SLUMS) and functional assessments.
- Exceptional interpersonal and communication skills to engage with patients, families, and care teams effectively.
Preferred Qualifications
- Training or certification in cognitive rehabilitation or memory care programs.
- Familiarity with electronic medical records (EMRs) commonly used in nursing home settings.
- Master's or Doctoral Degree in Nursing with a specialization in geriatrics or mental health.
