Job Summary: This position requires an individual who is especially detailed-oriented and has strong written, communication skills, and counseling skills, particularly in trauma-informed and client-focused practices. The selected candidate should be comfortable identifying and pursuing creative problem solving that is necessary for survivors of domestic violence. Job duties include but are not limited to conducting intakes, administering assessments, providing or seeing to the provision of necessary social services for survivors of domestic violence, providing housing assistance to all clients, including clients transitioning from shelter and collaborate with the Safe Passage team to ensure that all survivors receive quality services.
Duties and Responsibilities:
· Assist individual clients with obtaining a security alert system
· Enter all information in the NYC HRA IRM system
· Provide direct case management and support to individuals on caseload.
· Assist individuals with flex funding applications.
· Attend and participate in internal and external meetings.
· Refer clients to safe passage and Children Adolescent Servcies (CAS) programs.
· Provide housing assistance to all clients, including clients transitioning from shelter.
· Assist survivors with housing vouchers, Section 8, City FEPS/FHEPS, NYCHA.
· Assist in finding permanent housing and safety transfers.
· Dispute lease agreements with landlords and building management.
· Establish linkages with domestic violence shelters so that families discharged from shelters are supported in their new community.
· Directly provide (or give referrals for) homeless prevention services.
· Address 24 hours helpline crisis calls and walk-ins, documenting counseling and other services provided.
· Conduct comprehensive assessments with all clients and refer children/individuals/families to supportive and concrete services such as housing assistance, financial assistance, education, substance use, mental health, medical, immigration, legal, etc.
· Conduct assessments within 30 days of intake and review assessments with clients every three months after intake until goals are met, and the case is closed.
· Provide supportive counseling, case management, and crisis intervention. Work collaboratively with survivors to define case goals, objectives, and action plans.
· Meet program expectations for client contacts. Will contact client within 24 hours due to the critical nature of the work.
· Advocate on behalf of clients to overcome systemic barriers related to court, immigration, housing, public benefits, etc.
· Facilitate/co-facilitate groups for clients.
· Develop content, schedule, and participate in monthly outreach events and document attendance of participants.
· Collaborate with internal and external partners to advocate for survivors in court matters, housing matters, public assistance, immigration, etc.
· Complete all data collecting and reporting promptly. Document and maintain all required tracking logs, excel sheets, electronic data systems, and progress notes.
· Participate in meetings, assist in planning for client recreational activities and attend activities.
· Participate in weekly supervision with a planned agenda related to case issues.
· Implement Quality Assurance data to drive case practice, improve performance, and achieve outcomes.
· Participate in program development and agency required trainings on trauma-informed care, DBT, CBT, etc.

· Ability to conduct interviews with survivors to obtain and provide the information needed to carry out case management responsibilities.
· Knowledge and ability to use critical thinking and analytical skills used to formulate plans that are attainable in collaboration with the survivor.
· Ability to perform the job's essential functions with or without reasonable accommodation.
· Ability to collaborate and facilitate skills that work with individuals, families, and services providers.
· Demonstrates cultural sensitivity and competence skills in working with families from various ethnic and cultural groups, social-economic status, and lifestyles.
· Ability to express self effectively in written form.
· Ability to work across various computer systems and manage large data sets.
