Job Summary:

The Home Health Aide (HHA) provides direct patient care under the supervision of the RN Case Manager, contributing to the delivery of safe, high-quality home healthcare services. The HHA follows an individualized care plan and supports activities of daily living to ensure patient comfort and well-being.

Key Responsibilities

· Assist patients with personal care including bathing, grooming, toileting, oral hygiene, nail care, and dressing.

· Support mobility through ambulation assistance, transfers, and use of mechanical lift devices.

· Perform light housekeeping, meal preparation, and bed making.

· Monitor and report vital signs (temperature, pulse, respiration).

· Remind patients to take prescribed medications.

· Provide emotional support and companionship.

· Observe and report changes in the patient's condition to the RN Case Manager.

· Document care and interactions per agency policies.

· Maintain a clean, safe, and healthy environment for patients.

Qualifications/Requirements

· Minimum 18 years of age.

· Active HHA certification or Certified Nurse Assistant license; or willingness to complete HHA certification within 90 days of hire.

· Reliable transportation and communication methods.

· Valid driver's license and vehicle insurance (if transporting clients).

· Ability to pass criminal background and FBI biometric checks.

· Physical capability to perform lifting (up to 75 pounds), extensive bending, and standing for up to 6 hours daily.

· Demonstrated empathy, maturity, and effective communication skills.

· Basic computer literacy and office task familiarity.

· Completion of at least 12 hours of in-service training annually.

· Competency in patient observation and reporting.

· Ability to follow written and verbal instructions accurately.

· Maintain confidentiality and comply with all agency policies.

Physical Requirements

· Visual and auditory capacity to interact with patients and team.

· Ability to lift, bend, kneel, and perform physical tasks as needed.

· Minimum commitment of 8 hours per month, including at least 2 weekend shifts per month for permanent weekend cases.

· Annual documentation of clear TB test or chest x-ray.

Additional Expectations

· Pass competency evaluations with at least 70%.

· Participate in Quality Assurance Performance Improvement (QAPI) activities.

· Perform only those tasks permitted under state law and agency policy.
