Location: Trenton, MO (Serving Chariton, Linn, Livingston, and Grundy Counties)

Schedule: Flexible | Hybrid-Remote

Are you passionate about making a difference in your community? Preferred Family Healthcare is seeking a dedicated Community Support Specialist to provide compassionate, person-centered support to individuals across Chariton, Linn, Livingston, and Grundy counties. This role offers the flexibility of a hybrid-remote schedule and the opportunity to work closely with local communities to promote wellness, independence, and access to essential services.

In this role, you'll provide personalized, community-based support to clients in their homes, clinics, and other local settings. You'll serve as a mentor, advocate, and guide—helping clients build skills, overcome obstacles, and achieve their goals.

Key Responsibilities

  • Deliver Life-Changing Support

  • Provide compassionate, evidence-based mental health services in the community.

  • Empower Clients Through Behavioral Interventions

  • Use techniques like motivational interviewing, cognitive processing, and operant conditioning to support client progress.

  • Promote Recovery & Resilience

  • Engage clients in meaningful activities aligned with their treatment goals.

  • Manage a Balanced Caseload Collaborate with supervisors to maintain a manageable and impactful workload.

  • Collaborate on Care Plans Participate in team meetings to develop and refine individualized treatment plans.

  • Respond to Crises with Confidence Provide timely, appropriate support during mental health emergencies.

  • Ensure Continuity of Care Follow up on missed appointments, conduct outreach, and document all client interactions.

  • Grow with Us Attend trainings, workshops, and team meetings to stay current and expand your skills.

Education and/or Experience Qualifications

You may qualify through education, certification, or experience:

  • Bachelor's Degree in Psychology, Social Work, Education, Criminal Justice, Counseling, or related fields

OR

  • Associate of Applied Science in Behavioral Health Support

OR

  • 4+ Years of Experience in a related human services role (e.g., case management, residential support, vocational rehab)

OR

  • Qualified Addictions Professional (CADC, CRADC, CRAADC, CCJP, RADC, CCDP, etc.)

Additional Requirements

  • Valid driver's license and reliable transportation

  • Strong communication and interpersonal skills

  • Ability to work independently and collaboratively

  • Compassion, empathy, and a genuine desire to help others
