Hospice LVN

Position Summary:

The Hospice Licensed Vocational Nurse (LVN) provides skilled nursing care and support to patients and families receiving end-of-life care under the supervision of a Registered Nurse (RN) and in accordance with the hospice plan of care. The LVN helps ensure comfort, dignity, and quality of life for patients in their homes, facilities, or inpatient hospice settings.

Key Responsibilities:

• Provide direct patient care as outlined in the hospice plan of care, following physician and RN directives.
• Monitor patients' physical and emotional status and report changes promptly to the supervising RN or clinical manager.
• Administer medications and treatments as prescribed, ensuring accurate documentation of all care provided.
• Assist patients with activities of daily living (ADLs) such as bathing, dressing, and mobility.
• Educate patients and family members on comfort care techniques, medication administration, and symptom management.
• Maintain accurate and timely documentation in compliance with agency policies and state/federal regulations.
• Collaborate with interdisciplinary team members—including RNs, social workers, chaplains, and aides—to coordinate holistic care.
• Participate in on-call rotations as assigned, providing after-hours patient visits and support.
• Uphold infection control, safety, and hospice care standards at all times.

Qualifications:

• Current and valid Licensed Vocational Nurse (LVN) license in the state of practice.
• Minimum of one year of clinical nursing experience (hospice, home health, or palliative care preferred).
• Current CPR certification.
• Excellent communication, organization, and time-management skills.
• Compassionate, patient-centered approach with a commitment to end-of-life care principles.
• Valid driver's license, auto insurance, and reliable transportation (for home visits).

Work Environment:

• Work performed in patients' homes, assisted living facilities, or skilled nursing settings.
• Requires frequent driving and travel within assigned service areas.
• Exposure to various home environments and patient conditions.

Job Types: Part-time, PRN, Per diem

Pay: $ $60.00 per hour
