ResponsibilitiesPreventive Facilitator is responsible for facilitating family team conferences for all NYF Preventive Teams. Facilitators are a neutral party that works collaboratively with families, service providers and stakeholders in identifying strengths and developing plans to address safety and risk factors to impact treatment goals and progression.
Facilitate the gathering of family members, caregivers, service providers, and stakeholders to collaboratively develop a comprehensive plan for the care and well-being of the children and relevant family members, with the aim of promoting family safety and unity.
Foster a supportive environment where all participants can work together to address the child's needs, ensuring their safety, permanency, and overall well-being. This includes developing a tailored plan for each family that meets the specific needs of the family and ensures family voice and choice.
Coordinate Family Team Conferences, such as 30-45 Day Conferences, Preventive Planning Conferences, and Service Termination Conferences including inviting all relevant parties and ensuring interpreters are secured if applicable to ensure the conference is collaborative and supportive to the needs of the family, `
Conduct a thorough review of the case record in preparation for scheduled conferences and coordinate pre and post conference meetings with the assigned case planner to be aware of all safety and risk factors in advance and work collaboratively to ensure any case concerns are addressed.
Complete documentation within 48 hours of the conference and send copies of FTC Agreement Form to participants to support timely and effective manner in accordance with agency and stakeholder guidelines to ensure that all work is thoroughly documented, up-to date and accurate and all parties are aligned.

QualificationsBachelor's degree in social work, psychology, or a related field is preferred. Bachelor's degree in social work, psychology, or a related field with stakeholder approval.
Strong organizational and time management skills to plan and conduct Family Team Conference effectively.
Strong communication and interpersonal skills to effectively engage with family members, service providers, and stakeholders.
Strong ability to facilitate group meetings and coordinate collaborative efforts among diverse participants; cultural competence and sensitivity to work with families from diverse backgrounds.
Ability to work independently and part of a team
Previous experience working with children, families and/or social services.
Knowledge of child welfare policies and procedures is recommended
Ability to receive feedback to professionally grow and/or improve and be flexible with programmatic needs and changes
