Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients in various settings, including hospitals, nursing homes, assisted living facilities, and home care environments. As an LPN, you will work under the supervision of registered nurses and physicians, ensuring the highest standards of patient care while adhering to all medical protocols and regulations.

Responsibilities

  • Administer medications and treatments as prescribed by physicians.
  • Conduct patient assessments and monitor vital signs.
  • Assist in the development and implementation of patient care plans.
  • Provide personal care services, including bathing, grooming, and feeding.
  • Perform wound care and manage IV infusions.
  • Document patient information accurately in electronic health records (EHR) systems.
  • Collaborate with healthcare team members to coordinate patient care.
  • Educate patients and their families on health management and wellness strategies.
  • Maintain a clean and safe environment for patients.
  • Adhere to HIPAA regulations and ensure confidentiality of patient information.

Skills

Candidates should possess a variety of skills that enhance their ability to provide quality care. Preferred skills include:

  • Experience with Epic, Cerner, or Athenahealth EMR systems.
  • Critical care experience in Level I or Level II trauma centers.
  • Medical office experience with knowledge of ICD-9/ICD-10 coding.
  • Long-term care experience in assisted living or nursing home settings.
  • Proficiency in IV insertion, phlebotomy, catheterization, and medication administration.
  • Familiarity with acute pain management, hospice care, geriatrics, pediatrics, and emergency medicine.
  • Knowledge of anatomy, physiology, medical terminology, and diagnostic evaluation techniques.
  • Experience working with individuals with developmental disabilities or dementia care.
  • Skills in patient assessment, discharge planning, case management, and health coaching.
