Overview

We are seeking a dedicated and compassionate Patient Care Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring that patients receive the highest level of care and support throughout their healthcare journey. This position requires excellent communication skills, attention to detail, and a strong understanding of medical terminology and administrative processes.

Duties

  • Organize and manage caregiver schedules based on client care needs, staff availability, and geographic location.
  • Create and implement individualized care plans, ensuring all services are documented accurately and align with client goals.
  • Regularly review and update care plans in response to evolving health conditions, preferences, or feedback.
  • Monitor the quality of care through routine follow-ups, client feedback, and caregiver evaluations to ensure satisfaction and well-being.
  • Act as the primary liaison for clients, families, and caregivers, delivering responsive and compassionate support.
  • Match caregivers with clients by considering individual preferences, language compatibility, and any specialized care requirements.
  • Proactively adjust and maintain caregiver schedules to reflect changes in client care plans or caregiver availability.
  • Maintain consistent communication with caregivers to provide ongoing support, resolve issues, and ensure seamless service delivery.
  • Pre-billing
  • Spanish speaking a plus

Qualifications

  • Previous experience in a Home Care or medical office is highly preferred.
  • Strong organizational skills with the ability to manage multiple tasks simultaneously.
  • Excellent interpersonal skills to foster positive relationships with patients and team members.
