Partners in Home Care is actively seeking skilled and compassionate RN's to join our growing team in Toledo and surrounding areas. We offer flexible schedules with both per diem and part-time positions available.

Qualifications:

  • Must hold a current state license and/or registration (as required)
  • Completion of ongoing CEUs, training, and certification renewals
  • Valid driver's license and reliable transportation with a clean driving record
  • Dependable and committed to providing excellent care

Responsibilities:

  • Conduct home visits for Comprehensive assessments, reassessments, and discharge visits
  • Manage patient care and implement tailored treatment plans
  • Support patients and their families with expert guidance and care
