The Clinical Discharge Planner will provide clinical services within the psychiatric specialty area of adult inpatients. Performs a wide variety of social/clinical services for assigned client groups having complex social and adjustment problems and finds satisfactory ways to cope with their social situations and relationships by providing or supervising a wide range of psychiatric social casework/clinical services. Works in approved interdisciplinary treatment teams, these teams are staffed with psychiatrists; nurses; therapists; psychologists; nurse practitioners; among others. Performance of tasks requiring increasing insights into personality and into the interaction of society with persons having these problems. Interviews patients and their families if necessary, to obtain factual background information about the patient that require assistance. Summarizes information on the social and family background of patients and the history of the illness or disability to provide a basis for planning treatment; provides such information to those whose work with the patient requires it; works with community agencies to inventory community services and refer patients to them as appropriate.
- Master's degree in human services field and proven successfully developed skills in human services, crisis intervention, case coordination or discharge planning.
- Well developed critical thinking skills. Knowledge and understanding of medical terminology.
- Ability to communicate effectively with medical professionals of all disciplines.
- Outstanding professional, interpersonal, communication, and organizational skills. Ability to problem solve and meet daily deadlines. Multi-tasking and task management essential. Basic computer knowledge.
