Job Description:

We are seeking an experienced Healthcare Consultant with expertise in Patient-Centered Medical Home (PCMH) transformation to support primary care practices in achieving and sustaining PCMH recognition (e.g., NCQA, URAC). This role involves hands-on coaching, workflow redesign, data analysis, and quality improvement strategy implementation to enhance care coordination and align clinical practices with value-based care goals.

Key Responsibilities:

• Guide primary care practices through PCMH recognition and renewal processes (e.g., NCQA, URAC)
• Conduct practice assessments and gap analyses aligned with PCMH standards
• Develop and implement action plans to support quality improvement initiatives
• Provide training to providers and care teams on PCMH workflows, documentation, and team-based care
• Collaborate with cross-functional teams to integrate care coordination and population health strategies
• Support data tracking and reporting using EHRs and other health IT systems
• Stay up to date with CMS guidelines, HEDIS measures, and evolving healthcare regulations
• Foster strong relationships with clinical and administrative stakeholders

Qualifications:

• Minimum 3 years of experience in healthcare consulting, practice transformation, or quality improvement
• Direct experience supporting PCMH transformation, preferably with NCQA recognition processes
• Familiarity with value-based care models and population health management
• Proficient in EHR systems and data analysis tools
• Strong communication, facilitation, and stakeholder engagement skills
• Preferred: NCQA PCMH Certified Content Expert (CCE) certification will be plus.
