Job Description

Job Title: Physical Therapist - Long Term Care (LTC)

Job Summary:

We are seeking a dedicated Physical Therapist to join our team in providing exceptional care to patients in a Long Term Care facility. The ideal candidate will be compassionate, experienced, and committed to improving the quality of life for individuals in need of long-term care.

Responsibilities and Duties:

  1. Conduct evaluations and assessments on patients to determine their physical therapy needs.

  2. Develop personalized treatment plans and goals for each patient based on their unique needs and capabilities.

  3. Implement therapeutic exercises, manual therapy, and other interventions to improve patients' mobility, strength, and overall function.

  4. Collaborate with other healthcare professionals, including doctors, nurses, and social workers, to provide comprehensive care to patients.

  5. Educate patients and their families on proper techniques for at-home exercises and activities to promote continued progress.

  6. Maintain accurate and timely documentation of patient progress, treatment plans, and outcomes.

Qualifications and Skills:

  1. Valid license to practice as a Physical Therapist in the state of employment.

  2. Minimum of 1-2 years of experience working in a Long Term Care setting.

  3. Strong clinical skills and knowledge of evidence-based practice in physical therapy.

  4. Excellent communication and interpersonal skills to work effectively with patients, families, and other healthcare professionals.

  5. Ability to prioritize tasks, work independently, and adapt to changing patient needs.

  6. CPR certification is required.
