What You'll Do:

  • Perform home health nursing visits for a diverse patient population.
  • Complete comprehensive assessments and documentation using OASIS.
  • Develop, coordinate, and implement individualized patient care plans.
  • Communicate regularly with physicians, therapists, and family members.
  • Manage your caseload efficiently and independently while maintaining high-quality

clinical care.

What We're Looking For:

  • Licensed Registered Nurse (RN) in Michigan.
  • Strong clinical skills with a solid understanding of home health protocols.
  • Previous experience with OASIS documentation is required.
  • Excellent organizational, time management, and communication skills.
  • Ability to work independently while remaining highly collaborative with the care team.
