Job Summary:
The Behavioral Health Therapist will work as part of an interdisciplinary team of medical professionals and case managers. You will provide individual counseling to patients being seen in our healthcare clinic. The Mission of Heart Ministry Center is "Providing food, healthcare, and a way forward to people severely affected by poverty in the Omaha area". We do this through our free medical clinic, our food pantry, and our intensive case management and job training/placement program.
The Behavioral Health Therapist is responsible for providing diagnostic assessments, therapy, counseling, and support services to patients of the clinic. You will provide expertise and leadership in the development of clinical functions within the mental health treatment setting, serving as a resource to employees who may have mental health-related questions and require services. The therapist will address such issues as severe mental health issues, communication challenges, or family conflict. This is an opportunity to work as part of a dynamic multidisciplinary team, providing services to a broad spectrum of community members. The therapist will be familiar with other services and resources in the community and work closely with them to provide information and support when required.
Primary Functions:
- Provide evidence-based therapeutic and addiction treatment, ensuring a trauma-informed and recovery-focused approach according to professional standards and regulations.
- Facilitate sessions with individuals, groups, and families.
- Refer clients to community mental health professionals based on assessed needs.
- Evaluates and assesses the bio-psycho-social needs of patients and creates an individualized treatment plan based on the client's needs.
- Assists in determining the most appropriate level of care for the patient and makes appropriate referrals when indicated.
- Develops comprehensive treatment plans per policy and procedures.
- Serves as a resource for coordination and collaborates with clinical and contracted staff.
- Assists in the documentation of patient demographics and statistical data related to services provided.
- Constant communication with Division staff, as well as clinic and HMC employees, as needed.
- Crisis intervention as needed.
- Documentation reflects progress toward goals as established in the treatment plan.
- Actively participates in clinical and peer supervision to ensure quality of care and professional performance standards.
- Utilize expertise to assist in the development and implementation of policy and procedures, and identify opportunities for staff development.
- Mentor and lead group therapy sessions for Fresh Start Participants.
- Maintains confidentiality by HIPAA.
- Participates in interprofessional collaboration meetings
- Other duties as assigned.
Required Skills & Abilities:
- Detail-oriented and professional.
- Exceptional communication skills.
- Proficient with Microsoft Office Suite.
- Basic understanding of clerical procedures and systems, such as recordkeeping and filing.
- Ability to work independently and reliably.
- Flexible and adaptable in various situations and when interacting with many different personalities.
- Ability to organize and prioritize tasks, including delegation of tasks when appropriate.
- Knowledge of, or ability to quickly acquire, familiarity with the roles and functions of the assigned department, and any specifically applicable laws or guidelines.
Education & Experience:
- Master's degree in Counseling, Social Work, or Marriage & Family Therapy
- Two years of experience preferred
- Nebraska licensed required- LMHP or LIMHP requirements
- Experience treating clients with co-occurring mental and substance use disorders
- Bilingual, Spanish-speaking required
- Able to work with adults and children preferred
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
