Responsibilities

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
  • Monitor and observe patient health status, recording vital signs and noting any changes or concerns
  • Support medication administration following proper protocols and medical documentation standards
  • Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques
  • Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems accurately to document care activities and patient information
  • Assist with catheter care, wound management, and hospice or palliative care as needed
  • Use equipment such as Hoyer lifts safely to transfer patients and prevent injury for both caregiver and patient
  • Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation
  • Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment in compliance with HIPAA regulations
  • Support patients in community settings by helping with transportation arrangements or accompanying them to appointments

Qualifications

  • Proven experience in home health care, assisted living, nursing homes, or long-term care environments
  • Strong knowledge of medical terminology, vital signs measurement, medication administration, and patient safety protocols
  • Experience working with people with developmental disabilities or dementia-related conditions such as Alzheimer's disease
  • Ability to operate EMR/EHR systems efficiently for accurate medical documentation
  • Certified in CPR; additional certifications such as hospice care or catheter care are a plus
  • Familiarity with medical equipment including Hoyer lifts and other mobility aids
  • Compassionate caregiver with excellent communication skills and patience when working with diverse populations
  • Ability to work independently while following detailed care plans and adhering to HIPAA privacy standards
