Working under the supervision of a licensed clinical social work professional, provides information, education and social assistance to assigned individuals and families in the home, in hospital, and in congregate living facilities with regard to employment, finances, living facilities, and legal entitlements. Work may be specialized, such as, work with youth, the elderly, and other "at risk" groups.
Reports to the Director of Human Services Programs.
QualificationsExperience and Education:
A Master's degree from an accredited college or university in an applied behavioral science discipline, plus one year of related work experience*.
- Work experience obtained as a Social Service Intern while in an accredited Master's Degree program can be considered towards the one (1) year of related work experience.
Must possess and maintain a valid motor vehicle operator's license that enables the employee to drive in the State of Connecticut.
Qualifications:
Demonstrated knowledge of Federal, State and Municipal entitlement and benefit programs, as well as community resources available to individuals and families.
Working knowledge of principles, techniques and methods of social assistance delivery in both individual and group settings.
In-depth understanding of client confidentiality rights and personal data rights; proven knowledge of departmental policies and procedures of reporting and confidentiality
Demonstrated knowledge of how economic, cultural, and other social factors may influence attitudes and behavior of individuals and families, notably with the population groups in the operational area or program to which assigned.
Excellent written communication skills in English and Spanish, and the ability to prepare clear, concise, and accurate professional reports, case records, and correspondence in accordance with department requirements and standards.
Proven ability to communicate effectively in English and in Spanish, in both group presentations and as a professional advocate.
Demonstrated proficiency in the use of a personal computer with standard and industry-specific software programs and general office equipment.
Demonstrated ability to interpret and apply social service program policies. Proven ability to formulate appropriate, effective, measurable plans of action, and after supervisory review and approval, to implement applicable service plans in a timely manner.
Proven ability to work collaboratively as part of a team.
Demonstrated ability to deal tactfully, courteously, and harmoniously with people; ability to establish and maintain effective working relationships with clients, their families, staff, other agencies, and the general public.
Proven ability to manage time effectively, to work in complex or urgent situations, and to deal effectively with difficult individuals.
Adept ability to exercise discretion, restraint, and good judgment, with staff and clients; demonstrated ability to maintain the confidentiality of client information according to departmental policy and applicable state and federal laws; proven ability to motivate clients and encourage appropriate autonomy and action.

Demonstrated ability to conduct home or agency visits as required.
Examination: 100% Training & Experience Assessment
The examination to determine a candidate's eligibility for the position will be a Training and Experience Assessment. Duties and Responsibilities:
Obtains employment, housing, financial assistance, and other services for an assigned caseload, in accordance with local, state and federal laws and program guidelines. May direct the work of paraprofessional, clerical or volunteer staff as assigned.
Evaluates non-clinical client needs for assistance in regard to employment, finances, living facilities, and legal entitlements; Communicates with clients, their families, and outside agencies in order to determine technical eligibility for benefits and programs and assists clients in resolving problems with landlords and other key service providers.
Under the supervision of designated licensed professional, makes referrals to appropriate licensed professionals for assessment, diagnosis or treatment of bio-psychosocial dysfunction, disability or impairment, including mental, emotional, behavioral, developmental and addictive disorders.
Assists clients with completing applications and other documents/forms for State, Federal, local and private benefit programs and legal entitlements. Assists clients in presenting information at State, Federal and other proceedings. In accordance with department policies and procedures, handles financial matters such as calculating the eligibility of applicants for various financial assistance programs and provides limited assistance to clients with financial management and budgeting.
Conducts interviews, obtains client histories and reports to their supervising licensed clinical social work professional. With supervisory support, develops plan of action with measurable outcomes. Monitors client status and serves as advocate as appropriate, acting as a support representative for clients as required and as approved by supervisor. Provides resource recommendations along with procedural advice to individuals and groups to educate clients and family members, and to assist them to become more effective self-advocates. Participates in case conferences.
Confers with clients, relatives, clinical social work professional, and other social welfare agencies regarding client needs and problems. Consults with community groups to determine appropriate referrals and develop community resources.
Reports to and consults with supervisory licensed professional for referrals for medical evaluations, assessment, diagnosis or treatment of bio-psychosocial dysfunction, disability or impairment, including mental, emotional, behavioral, developmental and addictive disorders, counseling, psychotherapy, behavior modification or mental health consultation by licensed professionals.
Prepares and maintains mandatory client information, service activity data, forms and reports. Compiles statistics, on paper and on computer in a timely manner.
Conducts timely follow-up and reporting in accordance with departmental procedures, and policies.
Provides assistance to clients in department offices or via telephone as needed and/or conducts visits to community agencies, client homes, and congregate or convalescent homes.
May organize educational meetings, social functions, and group activities. May provide public information and education as authorized to community regarding programs and program needs. May serve on related community boards and committees as assigned.
Determines financial eligibility for program services and assists with calculating budget needs and expenses.
Provides limited after-hours service coverage as assigned. Without engaging in the practice of clinical social work as defined by law, may provide non-clinical patients with emergency crisis intervention and/or conflict mediation as authorized and in accordance with departmental policies and procedures. Contacts licensed social work professional for clinical patients. Ensures such cases receive expedited oversight review.
Maintains discretion and confidentiality in accordance with departmental policies and procedures.
Communicates and/or performs tasks in English and/or Spanish as necessary.
Performs related work as assigned.
Supplemental InformationAll positions may be subject to an interview process. The Town reserves the right to limit the number of qualified candidates who will be invited to participate in the examination, or any part thereof.
All outside candidates will be required to undertake and successfully pass a pre-employment medical examination, which includes a substance abuse test that includes testing for THC (the psychoactive ingredient in marijuana), given at the Town's expense prior to employment if conditionally offered a part time, full time or safety sensitive position.
The Town of Greenwich has implemented a policy of background investigations all full and part time positions prior to hiring as part of the reference checking procedures for outside candidates.
