Schedule: Monday – Friday | 8:00 AM – 5:00 PM | 40 Hours per Week | No Weekends or Overtime
Position Overview
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will deliver exceptional rehabilitative care, helping residents achieve their highest level of functional independence and quality of life. This role involves developing individualized therapy plans, coordinating interdisciplinary care, and maintaining strong communication with residents, families, and team members.
Key ResponsibilitiesClinical Duties
- Evaluate physician referrals and review residents' medical records to design customized Physical Therapy treatment programs.
- Perform thorough assessments and utilize advanced evaluation techniques to develop and implement personalized therapy interventions.
- Administer prescribed therapy programs using a variety of therapeutic modalities to restore function and prevent disability.
- Monitor, reassess, and adjust treatment plans to ensure ongoing progress toward established goals.
- Maintain accurate, timely documentation in accordance with professional and facility standards.
- Work closely with Occupational Therapy, Speech Therapy, and Nursing departments to coordinate patient care and maximize treatment outcomes.
- Clearly communicate treatment goals, progress, and discharge recommendations to residents, families, and interdisciplinary teams.
- Educate residents and caregivers on home exercise programs, safety measures, and continued rehabilitation practices.
- Supervise and mentor Physical Therapy Assistants (PTAs) to ensure quality care and efficient workflow.
- Conduct or participate in in-service training programs to enhance staff knowledge and performance.
- Support the ongoing professional development of the therapy team.
- Uphold the facility's mission, vision, and policies while maintaining the highest standards of resident care.
- Ensure confidentiality and safeguard resident information in accordance with HIPAA and facility regulations.
- Stay current with advancements in Physical Therapy through continuing education, workshops, and professional development.
- Demonstrate flexibility and enthusiasm when taking on additional responsibilities or projects as assigned.
- Graduate of an accredited Physical Therapy program.
- Valid Physical Therapist license in the state of [Confidential].
- One year of experience in acute care, long-term care, or rehabilitation preferred (new graduates are welcome to apply).
- Strong clinical reasoning and assessment skills.
- Excellent communication and interpersonal skills with the ability to adapt to diverse resident needs and personalities.
- Effective time management, prioritization, and multitasking abilities in a fast-paced environment.
- Competent in written documentation, problem-solving, and use of basic math skills.
- Proficient in Microsoft Office (Word, Excel, Outlook) and electronic health record (EHR) systems.
- Demonstrates professionalism, empathy, and commitment to patient-centered care.
- Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an accredited institution.
- Current state licensure as a Physical Therapist (PT)
