Mobile case management/provides outreach services

A. *Meets with youth in community locations, providing basic needs, resource materials, crisis support and counseling

B. *Identifies and assesses resource needs

C. *Maintains contact with Emergency Services, local homeless shelters, library, bus stops, ALC, Lake George area, Downtown, parks, woods/other areas

D. Coordinates and plans for emergency situations and referrals as needed

E. *Advocates for and protects rights of youth

F. *Supports homeless youth in their education

G. *Uses a trauma informed, strengths-based approach

I. Attends court hearings with youth when approved or required to testify

J. Provides additional documentation to the court or social service as requested

Responds to referrals coming to program and makes referrals when necessary to other programs

A. *Responds to referral phone calls coming into the Transitional Housing Program that may

need immediate response during work hours

B. *Maintains the referral list for the program and referral list for youth in the

regional Coordinated Entry System

C. Provides program information to those seeking it, general support, and high skill counseling

to clients when needed and refers to other staff when requested or appropriate

D. *Responds to calls for basic needs

E. *Responds and acts on calls from partners (county, schools, social workers, mental health

workers)

F. *Provides information and refers youth to community services

Performs screening/intake with new referrals and outreach youth, explaining services to client and completing all required paperwork

A. *Completes intake form, assessments, and required stakeholder paperwork

B. *Explains the program guidelines and rights and responsibilities

C. *Establishes eligibility for internal programming or referral to another program

Maintains professional relationships with community resources

A. *Has knowledge of area resources and can determine client's eligibility

B. *Meets with professionals to discuss and promote program

C. *Makes presentations to the community to raise awareness and develops referrals

D. *Participates in homeless committees such as: Continuum of Care (COC), Coordinated

Entry (CE), Central MN Human Trafficking Coalition, COC P.I.T., Central MN Youth

Collaboration, Community Connections, and Project Connect Planning Committee, or as

assigned

Data collection and documentation of client interactions

A. *Maintains case records to effectively document case activity and support case decisions

B. *Data Entry for preliminary paperwork in Statewide Software system and internal software

C. *Data entry for all contacts into internal software

D. *Complies with requirements in the Minnesota Data Practices Act

E. *Participates in Point In Time (P.I.T.) count

Adheres to the guidelines established by the program and by funders

A. *Attends required training and training offered pertinent to at-risk youth

B. *Advocates for teaching youth how to advocate for themselves

C. *Maintains transportation to and from client's home or community and occasionally

provides transportation for clients in an emergency with prior approval from Supervisor.

Education:

BA in Education, Social Work, Psychology, Community Psychology or related human services field.

Job-Related Experience:

2 years experience working with youth; prefer experience in homelessness/mental health/chemical health.

Additional Knowledge, Skills, and Abilities Required:

Must be at least 21 years old. Must have a valid driver's license and a working vehicle.

The Transitional Housing Homeless Outreach Worker position is responsible for connecting resources to youth who are homeless or at risk of homelessness and providing a nonjudgmental approach to helping solve their crisis. The Transitional Housing Homeless Outreach Worker provides mobile case management and counseling to youth in crisis on the streets.
