Position Overview
We are seeking an experienced Home Health Physical Therapist to join our Washington, DC team. This role offers autonomy, flexibility, and strong interdisciplinary support while delivering high-quality, patient-centered care in the home.
Key Responsibilities
Perform comprehensive physical therapy evaluations
Develop and implement individualized treatment plans
Provide skilled therapy to improve mobility, strength, balance, and function
Educate patients and caregivers on exercises and safety
Document visits accurately and timely in EMR
Collaborate with nurses, physicians, and care team members
Supervise Physical Therapist Assistants (PTAs)
Ensure compliance with Medicare, HIPAA, and agency policies
Qualifications
Graduate of an accredited Physical Therapy program
Active DC Physical Therapist license
Minimum 3 years of clinical experience (home health preferred)
OASIS experience strongly preferred
Proficient with EMR systems (WellSky/Kinnser or Homecare Homebase preferred)
Strong communication and clinical skills
Valid driver's license and reliable transportation
