View Point Health is a community behavioral health organization utilizing a team of psychiatrists, nurses, licensed counselors and social workers, as well as other professionals to provide services to individuals who need treatment and support to cope with mental illness, substance abuse, and intellectual and developmental disabilities. View Point Health serves uninsured, underinsured, low-income Medicaid, Medicare, war veterans and some private insurance across multiple locations with a full continuum of behavioral health services and support.

The Behavioral Health Case Manager provides outpatient adults with severe mental illness and/or substance use with additional support services to prevent a need for higher level of care (i.e., ACT, ICM, hospitalization). The services provided to outpatient children and adolescents with behavioral health challenges are Case Management (CM), Skills Training (PSR-I) and Community Services Individual (CSI).

Flexibility in scheduled hours is necessary to provide effective non-clinical-based services throughout the day and some evening hours, as needed. This position also requires you to work closely with a team of professional staff to assess needs, develop treatment plans, and monitor client progress.

Duties & Responsibilities:

  • Interviews client, family members, and/or support system(s) to assess level of functioning, current Strengths, Needs, Abilities, and Preferences and secure information relative to program requirements for receiving services and recommending appropriate level of care.
  • Complete Behavioral Health Assessments and update Treatment plans to reflect needed services for authorization.
  • Consult with licensed clinician to assess for hospitalization and completion of 1013, if warranted
  • Participate in monthly treatment team meetings to assist in the development of treatment plans with appropriate therapeutic interventions, notify professionals of any unusual observations or incidents, client progress, and follow up.
  • Develops and maintains a network of community contacts. Facilitate referrals and linkages to community services and communication among client's service providers with VPH to assure a continuum of services.
  • Provide linkage and assist/coach clients in accessing & obtaining community resources.
  • Establishes linkage with community resources (jails, hospitals, shelters, DFCS, etc.) to assist with discharge planning and a smooth transition to View Point Health Services.
  • Write precise, accurate, and clear progress notes in ECR/Carelogic in a timely manner.
  • Maintain required records and information in each client's file. Prepares accurate, complete timely documentation (within 24 hours) of case notes and billing information to meet agency and state standards and provide an effective delivery service across a broad range of services.
  • Provide individual skills training as indicated in the client's treatment plan including but not limited to independent living skills, coping skills, interview skills, symptom management skills, anger management skills, assertiveness skills, advocacy skills, hygiene skills, and many other daily living skills.
  • Recognize the need for crisis intervention and work with licensed professional to assess for higher need of care. Notifies security, supervisor, and/or professional staff of escalated situations.
  • Communicate and work closely with family members to support individuals in crisis. May assist with coordination of transportation, completion of all required documentation and referrals for admission to the CSUs and local hospitals.
  • Provides individual and group skills training and support clients and families concerning their needs and problems, and issues with personal and social development.
  • Maintain accurate mileage log and sign in sheet.
  • Will work directly with children, adolescents, caregivers, family members, school staff, community stakeholders and partners through in-home visits (C&A) and in-school visits (C&A)
  • Maintain update community resources for school and clients
  • Meet with school counselors to update cases (C&A).
  • Assesses and addresses school related issues: behavior, attendance, absenteeism, and barriers.
  • Establishes and maintains relationships with other community providers in outreach services, school activities, community fairs.
  • Links caregivers with community resources according to their needs (financial, job, training, Medicaid, food stamps).
  • Provides strength-based interventions and developing skills that are directly associated with the client's individual target behavior and related back to their treatment plan.
  • Maintain update authorizations for billing and notifying clinicians when authorizations are needed.
  • Other related duties as assigned

Programming Specific Duties:

  • Case Manager(s) are expected to carry a caseload of no more than 50 clients
  • Contact must be made with the individual receiving CM or PSR-I a minimum of two (2) times a month. At least one of the monthly contacts must be face-to-face in a non-clinic/community-based setting and the other may be either face-to-face or telephone contact depending on the individual's identified support needs
  • At least 50% of CM or PSR-I service units must be delivered face-to-face with the identified individual receiving the service and the majority of all face-to-face service units must be delivered in non-clinic settings over the authorization period.
  • In the absence of meeting the minimum monthly face-to-face contact and if at least two (2) unsuccessful attempts to make face-to-face contact have been tried and documented, the provider may bill for a maximum of one (1) telephone contact in that specified. Billing for collateral contact only may not exceed 30 consecutive days.
  • Spends approximately 50% of total scheduled work hours in the community, for example, meets with clients in the community.

Minimum Qualifications:

Bachelor's degree in counseling or a related field from an accredited college or university and two (2) years of job-related experience; or one (1) year of experience at the lower-level Behavioral Health Counselor 1 (HCP100) or position equivalent. Preferred Qualifications:

  • Master's degree in a related field from an accredited college or university
  • Ability to actively listen, clearly communicate, and build rapport with individuals in crisis situations.
  • Knowledge of crisis intervention techniques, including de-escalation strategies, safety planning, and emotional regulation strategies.
  • Understanding of common mental health issues and their symptoms, as well as crisis situations related to trauma, abuse, or substance use.
  • The ability to connect with individuals experiencing distress and provide genuine support.
  • The Ability to assess situation quickly, identify appropriate interventions, and make informed decisions in crisis situations.
  • Training Specific to Individual Placement & Support model
  • At least 1 year of experience working with youth and young adults at transition age
  • At least 1 year of experience working with SPMI (Severe Persistent Mental Illness) population

Requirements/Competencies:

  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting).
  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities
