This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.
 
Licensed Practical Nurse (LPN) – Adult Day Care

Location: Dorchester, MA
Schedule: Monday–Friday | 7:00AM–3:00PM or 8:00AM–4:00PM
Salary: $30–$33/hour
Bilingual Preferred: Haitian Creole / English

No weekends. No evenings. No on-call shifts

Sevita is excited to announce an opportunity for a Licensed Practical Nurse (LPN) to join our Adult Day Health team in Dorchester We are seeking a reliable, compassionate nursing professional with a strong clinical background and a commitment to providing high-quality care.

Key Responsibilities
  • Monitor the health and well-being of program participants, ensuring the highest quality of care.

  • Administer medications, maintain accurate documentation, and oversee staff who assist with medication administration.

  • Provide prescribed medical treatments and personal care in accordance with physician's orders.

  • Conduct assessments, develop and implement health care plans, and respond appropriately to changes in condition.

  • Train staff on topics such as infection control, universal precautions, and medication administration.

  • Serve as a liaison and advocate between participants, families, physicians, and community service providers.

  • Educate participants on topics such as self-medication, smoking cessation, and STD prevention.


Qualifications
  • Current Massachusetts LPN or LVN licensure required.

  • Strong assessment, communication, and clinical documentation skills.

  • Ability to work collaboratively with staff, participants, and families.

  • Prior experience in an adult day health, community health, or long-term care setting preferred.

  • Bilingual in Haitian Creole preferred.
