  • Arrowhead Congregate Living Health Facility LLC – Colton, CA

Unlike large SNFs, our facilities maintain a 6–10 patient ratio per nurse, supported by a full team including RNs, LVNs, CNAs, RNAs, activity staff, cooks, and a social worker. This allows you to provide more focused and meaningful care while working in a supportive environment where staff collaboration is valued.

Requirements:

  • Active California CNA certification in good standing
  • Must be available for some weekend shifts
  • Current BLS certification (required)
  • Prior experience in congregate living, SNF, or similar care settings (preferred)
  • Strong teamwork skills and ability to follow care plans under nurse supervision

Available Shifts:

  • Day Shift: 6:00 AM - 6:30 PM
  • Noc Shift: 6:00PM - 6:30AM
  • VARIED DAYS

Responsibilities:

  • Provide personal care and assistance with activities of daily living (ADLs), including bathing, dressing, grooming, feeding, and toileting
  • Take and record vital signs, weights, and intake/output accurately
  • Assist residents with mobility and positioning, including use of lifts and transfer devices
  • Maintain accurate documentation and promptly report changes in resident condition to the nurse
  • Support residents' emotional and social well-being by fostering dignity, respect, and engagement
  • Collaborate with RNs, LVNs, RNAs, and the interdisciplinary team to ensure quality care
