Job Details
Position Title: Supported Living Specialist (SLS)
SUMMARY:
The Supported Living Specialist provides and/or coordinates supports that are necessary to enable FRA participants to live as independently as possible. This includes assisting in the planning, implementing and documenting of the Plans of Care (POC's), ensuring the well-being, safety and quality of life of program participants, providing guidance and training that facilitates independence and recognizing and respecting the rights of the participant.
Status: Full Time, Part Time, and On-call
Full time SLS scheduled shifts, shifts are three days and alternate Wednesday every other week:
Day Shifts:
- Sunday-Tuesday, Wednesday biweekly 6:45am - 7pm
- Wednesday biweekly, Thursday - Saturday 6:45am - 7pm
Night Shifts:
- Sunday-Tuesday, Wednesday biweekly 6:45pm - 7am
- Wednesday biweekly, Thursday - Saturday 6:45pm - 7am
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- Demonstrate a personal commitment to the development and implementation of the least restrictive residential services for participants.
- Demonstrated skill in implementing and documenting direct participant training. Administrative and management skills necessary to supervise the day-to-day activities of residential participants.
- Flexible scheduling is necessary.
- Must have reliable transportation and maintain a good driving record.
- Must possess the knowledge and ability to utilize a computer and electronic medical record (EMR) system.
- Ability to learn new software and hardware applications.
- Must have sufficient language skills to meet the needs of participants and the ability to access emergency services, including giving clear directions to emergency personnel of the home's location and describing the nature of the emergency in the primary language of the community.
Minimum Requirements:
- H.S. Diploma or Equivalent.
- 21 years of age.
- Must have a valid Driver's License.
- Must pass a thorough background check conducted by the State of Alaska Background Check department.
- Must be able to fulfill the physical aspects of the position, including but not limited to the ability to physically restrain clients as needed.
- Must be willing to work a Flexible work schedule, including weekends.
- Must have basic computer and typing skills.
IMMUNIZATION REQUIRED:
- Annual PPD Test
- Hepatitis B immunization (or) Waiver
- Fully Covid-19 vaccinated or medical/religious exemption
LICENSES/CERTIFICATIONS REQUIRED:
- Alaska Driver's License
Agency Provided Certifications
- Certification in Adult/Child and Infant CPR
- Certification in First Aid
- Safety-Care Certification
WORKING CONDITIONS:
Medium Work: Exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
PHYSICAL ACTIVITIES:
- Lifting: Raising objects from a lower to a higher position or moving objects horizontally from position-to-position.
- Talking: Expressing or exchanging ideas by means of the spoken word. Those activities which demand detailed or important instructions spoken to other workers accurately, loudly or quickly.
- Driving: Minimum standards required by State Law (including license).
ENVIRONMENTAL CONDITIONS:
None: The worker is not substantially exposed to adverse environmental conditions.
