Job Summary

We are seeking an experienced and dedicated Home Health Supervisor to oversee clinical operations and ensure the delivery of high-quality patient care in a home health setting. The ideal candidate will possess extensive nursing and healthcare management experience, with a strong background in various specialties such as geriatrics, behavioral health, pediatrics, and acute care. This leadership role involves supervising clinical staff, managing patient assessments, ensuring compliance with healthcare regulations, and coordinating interdisciplinary care teams. The Home Health Supervisor will play a vital role in maintaining operational excellence while fostering a compassionate environment for patients with diverse needs, including those requiring dementia care, memory care, or assistance with developmental disabilities.

Duties

  • Oversee daily home health operations, ensuring adherence to clinical protocols and regulatory standards such as HIPAA and NCQA standards.
  • Supervise nursing staff and interdisciplinary teams, including nurse management, case management, and support personnel.
  • Conduct patient assessments, including vital signs, physical examinations, and medication administration.
  • Coordinate discharge planning, treatment plans, and ongoing care strategies tailored to individual patient needs.
  • Ensure proper documentation using EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks.
  • Manage medical records, medical documentation, and coding procedures including ICD-9/ICD-10/CPT coding.
  • Oversee infection control practices and aseptic techniques during procedures such as catheterization or sterile processing.
  • Provide guidance on complex clinical procedures including IV infusion/insertion, ventilator management, tube feeding, spinal tap, and wound suturing.
  • Monitor patient safety through airway management techniques and pain management protocols.
  • Facilitate staff training on new technologies or procedures such as DME (Durable Medical Equipment), laboratory specimen collection/processing, or diagnostic evaluations.
  • Lead quality assurance initiatives by conducting utilization review and utilization management activities.
  • Ensure compliance with legal requirements related to workers' compensation law and occupational health standards.
  • Collaborate with physicians during hospital medicine rounds or Level I/II trauma center cases when applicable.
  • Promote a culture of continuous improvement in patient care quality and staff development.

Qualifications

  • Valid nursing license (RN or higher) with current registration.
  • Extensive experience in home health nursing or related fields such as medical-surgical, ICU, emergency medicine, or primary care.
  • Proven supervisory or nurse management experience in healthcare settings.
  • Strong knowledge of EMR/EHR systems including Epic, Cerner, Athenahealth, eClinicalWorks; familiarity with HIPAA regulations.
  • Experience working with diverse populations including geriatrics (memory/dementia care), pediatrics (neonatal ventilator), behavioral health, or individuals with disabilities.
  • Proficiency in medical terminology and physiology relevant to patient assessment and diagnostic evaluation.
  • Skills in medication administration, injections (including phlebotomy), specimen collection/processing.
  • Ability to perform physical examinations and vital signs accurately.
  • Knowledge of infection control practices and aseptic techniques.
  • Excellent leadership skills with the ability to manage multidisciplinary teams effectively.
  • Strong communication skills for triage situations and family education.
  • Certification in Basic Life Support (BLS) required; additional certifications such as IV insertion or catheter care are preferred.
