Five Rivers Health Centers is an expanding Federally Qualified Health Center (FQHC) providing a medical home for low-income patients in Dayton and Xenia, Ohio. We provide quality medical, dental, vision, and behavioral healthcare to over 27,000 patients with more than 90,000 visits annually at our seven convenient locations.

Position Overview
The Community Health Worker (CHW) will support the mission and vision of Five Rivers Health Centers by conducting outreach in the community and working directly with patients to help them navigate and access community services while adopting healthy behaviors.
General Responsibilities
- Assess the patient's needs by interviewing and reviewing medical records
- Provide information on available and appropriate community resources to help overcome barriers.
- Assist with or coordinate patient transportation services
- Provide social support and advocate for the health needs of the patient and community.
- Assist with power of attorney, living will, and utility paperwork.
- All tasks will be completed, as well as any other duties as assigned, in compliance with Five Rivers Health Center's policies and procedures.
Qualifications
- High School diploma or equivalent required
- Associates Degree in the healthcare or business field preferred
- 2-3 years of health/social services experience preferred
- Windows-based computer experience required
