Job Overview

We are seeking a dynamic and compassionate Hospice Clinical Nurse Manager to lead our hospice care team with energy, expertise, and a commitment to excellence. Responsibilities

  • Lead and supervise a multidisciplinary hospice care team, including registered nurses, social workers, chaplains, and support staff, ensuring high standards of patient-centered care.
  • Oversee patient assessments, ensuring comprehensive evaluations that include vital signs, medical history, physiology knowledge, and symptom management.
  • Coordinate interdisciplinary care plans that incorporate pain management, dementia care, Alzheimer's care, memory care, and acute pain management strategies tailored to each patient's needs.
  • Manage clinical workflows within electronic health record (EHR) systems such as Epic or Cerner, ensuring accurate medical documentation and compliance with HIPAA regulations.
  • Oversee medication administration protocols including IV infusion, catheterization, tube feeding, wound care (including suturing), and ventilator management where applicable.
  • Conduct regular audits of medical records and documentation to ensure adherence to CHAP standards and proper ICD-10/ICD-9 coding practices for billing and reporting purposes.
  • Facilitate discharge planning and family education sessions on topics such as nutrition, infection control, infection prevention techniques like aseptic technique, and advanced directives.
  • Implement quality improvement initiatives focused on patient safety, infection control practices, utilization review, utilization management, and case management processes.
  • Ensure staff are trained in emergency procedures including airway management, basic life support (BLS), ventilator use, spinal tap procedures when necessary, and sterile processing techniques.
  • Promote a collaborative team environment by supervising staff performance through coaching, evaluations, and ongoing professional development in areas such as geriatrics, behavioral health, pediatric care including neonatal ventilator management and toddler care.
  • Maintain compliance with all regulatory requirements related to hospice services while fostering a culture of continuous improvement aligned with NCQA standards.

Experience

  • Proven experience in Hospice and Care Delivery to patients and families in end of life.
  • Demonstrated leadership experience in nurse management or supervisory roles within hospice or palliative care settings.
  • Extensive background working with diverse populations including geriatrics, patients with developmental disabilities or disabilities requiring specialized nursing interventions such as DME (durable medical equipment) use or working with people with disabilities.
  • Familiarity with EMR/EHR systems such as Epic or Cerner is essential; experience with Athenahealth or eClinicalWorks is a plus.
  • Strong knowledge of medical terminology related to cardiology (cardiac catheterization), ophthalmology (ophthalmology procedures), sonography (medical imaging), laboratory experience including specimen collection/processing and phlebotomy.
  • Skilled in managing complex cases involving ICD coding (ICD-10/ICD-9), CPT coding for procedures like gastric lavage or spinal tap; experience in utilization review/management is highly desirable.
  • Knowledge of nursing practices related to IV insertion/injection administration, catheter care (including Foley catheters), tube feeding techniques, medication administration protocols for acute pain management or sedation when necessary.
  • Ability to work effectively with multidisciplinary teams across various healthcare settings including assisted living facilities and long-term care environments such as nursing homes or skilled nursing facilities. Pay: $115, $130,000.00 per year
