Purpose of Your Job Position

The primary purpose of your job position is to assist in the administering of medications to residems as ordered by the attending physician. under the direction of the attending physician. the nurse supervisor or charge nurse. and the Director of Nursing Services. The administration of medications shall be in accordance with established nursing standards, the policies. procedures, and practices of th is facility, and the requirements of this state.

Administrative Functions

  • follow the policies and procedures of the facility governing the administering or medications lo residents.
  • Note and report errors in the administration of medications to Supervisor
  • Implement procedures or programs of the facility that seek to determine medication errors to include if they are taking place. when, where, and whythey occur, and how they can be prevented.
  • Administer all medications and treatments only within licensure/cenification.
  • Perform administrative requirements. such as completing necessary forms, charts, reports, etc., and submit these as may be required.
  • Maintain a friendly, productive, working relationship with other members of the department of nursing, attending physicians, the Medical Director, and pharmacists.
  • Report any complaints or grievances and possible resolutions made by residents to the Director ofNursing Services.
  • Participate and cooperate with any facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director ofNursing Services.
  • Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Report any known or suspected unauthorized attempt to access facility's information system.

Committe Functions

  • Participate in Quality Assurance and Performance Improvement programs. the Pharmaceutical Services Committee, and any other facility commil1ee or program that afTects the medication pass and seeks lo improve the performance and accuracy or the medication process as directed.
  • Attend and participate in care plan meetings as necessary.

Drug Administration Function

  • Accurately and safely prepare, administer, and document the oral and/or topical medications that arc commonly used in this facility and that may be ordered for resident use by the attending physician or the Medical Director.
  • Verify that any medications brought into the facility by a newly admitted resident are examined and identified by the attending physician or the facility pharmacy, pharmacist or Charge Nurse.
  • Follow the labeling policies and practices of the facility.
  • Make certain that all medications administered arc properly labeled.
  • Assist with medication charting as directed by the Nurse Supervisor/Charge Nurse.
  • Administer medications and treatments according to standard nursing practice. using 5 rights to ensure resident safety.
  • Follow facility procedures in regard to charting or medications.
  • Follow facility policies and procedures regarding the destroying of medications; assist in carrying out these rules under the direction of the facility pharmacist or Director of Nursing Services. including the documentation requirements.
  • Assist in documenting and removing medications that are discontinued by the attending physician.
  • Follow facility procedures governing drug receipt errors, including notifying the delivery source and the Director ofNursing Services.
  • Ensure that medication is returned and reordered except emergency drngs.
  • Ensure that documentation of the drug receipt error includes a wrirten error report, signed by an observer/witness and you.
  • Follow faciIity procedures in holding medications for residents who are transferred from the facility or when the attending physician discontinues medications.
  • Accurately measure, record, and report the vital signs or residents.
  • Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered. including rcason(s) for refusal.
  • Promptly notify the Charge Nurse, Nurse Supervisor. or Director of Nursing Services of any errors or reactions to medications by residents.
  • Follow the medication card practices of the facility.
  • Ensure that no medication is administered without a written, signed. dated order from an attending licensed physician.
  • Follow the facility procedure for recording the order in the resident's clinical record, including the type, route. dosage, frequency and strength of each medication.
  • Verify the identity of the resident before administering the medication treatment.
  • Follow the facility's procedures allowing residents lo self-administer his or her medications.
  • Assist the Nurse Supervisor/Charge Nurse in monitoring and documenting all aspects of such self-administering.
  • Promptly notify the Nurse Supervisor/Charge Nurse of any failure of a resident to properly take any ..self-administered" medications or refusal to take medications. Document as appropriate.
  • Follow established guidelines concerning the storage of drngs and biologicals.
  • Be familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the facility, pharmacists, and physicians.
  • Participate in the develop111ent and i111plc111emation of the resident's comprehensive assessment as necessary.
  • Others related duties as may become necessary or as directed by your supervisor.

Personnel Functions

  • Report known or suspected incidents of fraud to the Administrator.
  • Make certain that departmental computer workstations leR unattended are properly logged off or the password protected automatic screen-saver activates within established facility policy guidelines.
  • Direct certified nursing assistants with re. 10 resident care as appropriate. Report care concerns or resident status changes lo Charge Nurse.

Staff Development

  • Attend and participate in continuing educaiion programs designed to keep you abreast of changes in your profession. as well as lo maintain your certification on a current status.
  • Attend annual in-service training programs as scheduled (e.g., OSI-IA, TB, 1-HIPAA, Abuse Prevention, Safety, Lnfection Control, etc.).
  • Attend and participate in orientation programs as necessary.

Safety and Sanitation

  • Use personal protective equipment when performing procedures that involve exposure 10 blood or body fluids.
  • Report all hazardous conditions and equip111en1 10 the nurse supervisor/charge nurse immediately.
  • Follow established safety precautions in the performance of all duties.
  • Wash hands before and aRer perfor111ing any service for the resident.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents.
  • Practice infection control, to include good hand washing, and appropriate handling of infectious materials per facility policy.

Resident Rights

  • Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
  • Report any complaints, grievances and possible resolutions made by residents to the Director ofNursing Services.
  • Knock before entering the resident's room.
  • Report incidents or suspected incidents of resident abuse to the DON immediately.
  • Honor the resident's refusal of treatments or medications request.
  • Report a resident's refusal of treatments or medications 10 the nurse supervisor/charge nurse.

Education

Must possess, as a minimum, a high school education or its equivalent.

Experience

Must be a licensed Certified Medication Aide having successfully completed a state approved training program and any necessary examination(s) in accordance with laws of this State.

Specific Requirements

Must be able to read, write, speak, and understand the English language.

Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served. Must be a supportive team member, contribute to and be an example of team work and team concept.

Must possess the ability to make independent decisions when circumstances warrant such action.

Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

Must possess the ability and willingness to work harmoniously with and other personnel.

Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. Must be able to relate information concerning a resident's condition.

Must provide documentation of such certification upon application for the position.

Must not pose a direct threat to the health or safety of other individuals in the workplace.

Physical and Sensory Requirements

(With or Without the Aid of Mechanical Devices)

Must be able to move intermittently throughout the workday.

Must be able to speak and write the English language in an understandable manner. Must be able to cope with the mental and emotional stress of the position.

Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.

Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.

Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.

Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.

May be necessary to assist in the evacuation of residents during emergency situations.
