Position: RN-Case Manager (Travel/Contract)

We're hiring experienced RN-Case Manager for a 14-week contract in Baltimore, Maryland — earn up to ($2035 - $2143 per week) while providing essential care at a leading facility. Most candidates will need to have at least 2 years of recent experience in the specialty.

Responsibilities

  • Manage patient care plans and coordinate services for optimal outcomes.
  • Conduct assessments to identify patient needs and develop care strategies.
  • Collaborate with healthcare teams to ensure comprehensive patient support.
  • Monitor patient progress and adjust care plans as necessary.
  • Educate patients and families on care plans and health management.
  • Utilize data to track performance metrics and improve service delivery.
  • Participate in professional development opportunities to enhance skills.
  • Ensure compliance with healthcare regulations and standards.
  • Maintain accurate documentation of patient interactions and care plans.
  • Engage in continuous learning to stay updated on best practices.
  • Develop and implement strategies to achieve performance targets.
  • Provide support and guidance to junior staff and new team members.

Experience/Qualifications

  • Registered Nurse (RN) license in Maryland or compact state required.
  • Commitment to continuous learning and professional development.
  • Experience in case management and patient care coordination is essential.
  • Proven track record of meeting performance targets and goals.
  • Strong communication skills to educate patients and collaborate with teams.
  • Knowledge of healthcare regulations and compliance standards.
  • Ability to analyze data for performance metrics and quality improvement.
  • Ability to adapt care plans based on ongoing patient assessments.
  • Familiarity with electronic health records and documentation practices.
  • Experience mentoring or supporting junior staff is a plus.
