Position Summary

The Patient Care Coordinator serves as a key liaison between patients, healthcare providers, and administrative staff. This role ensures that patients receive timely, compassionate, and well-organized care by managing appointments, coordinating services, handling insurance authorizations, and providing clear communication throughout the care process. The ideal candidate is empathetic, detail-oriented, and highly organized with strong interpersonal and customer service skills.


The successful candidate will be responsible for the following:
Key Responsibilities:
  • Serve as the primary point of contact for patients regarding appointments, follow-ups, and care plans
  • Schedule, confirm, and coordinate patient appointments, tests, and procedures
  • Ensure patients understand their treatment plans, medications, and provider instructions
  • Communicate with medical staff, specialists, and other healthcare professionals to coordinate comprehensive care
  • Assist with insurance verification, pre-authorizations, and referrals
  • Maintain accurate and up-to-date patient records in the EMR/EHR system
  • Monitor patient progress and follow up as needed to ensure continuity of care
  • Respond to patient inquiries, complaints, and concerns with professionalism and empathy
  • Support quality assurance initiatives and patient satisfaction goals
  • Educate patients on clinic policies, forms, and documentation

Qualifications:
  • High school diploma or GED required; Associate's or Bachelor's degree in healthcare, business, or a related field preferred
  • 2+ years of experience in a healthcare or clinical setting, preferably in a patient-facing role
  • Familiarity with medical terminology and health insurance processes
  • Experience with electronic medical records EMR: Athena Preferred
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management abilities
  • Compassionate, patient-focused attitude
  • Bilingual skills are a plus

Preferred Skills:
  • Knowledge of HIPAA and healthcare regulations
  • Background in nursing, medical assisting, or case management is a plus
  • Ability to multitask in a fast-paced clinical environment
  • Proficiency with Microsoft Office and scheduling software
