Position Overview

·
• 80% Field Time / 20% Office Time

·
• Conduct in-home assessments to evaluate clients' physical, emotional, psychosocial, and environmental needs.

·
• Develop comprehensive care plans that address Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).

·
• Collaborate closely with an RN and Senior Care Coordinator to ensure individualized, high-quality care for every client.

·
• Maintain accurate and timely documentation.

Qualifications

·
• Active Licensed Social Worker (LSW or LCSW) in the State of Louisiana.

·
• Experience in Home Health, Hospice, or Case Management (hospital or community setting preferred).

·
• Strong assessment, documentation, and organizational skills — detail-oriented and dependable.

·
• Proven experience working with the senior population.

·
• Ability to work independently in the field and effectively communicate within a team.

·
• Positive energy, professionalism, and compassion for helping others.
