Position Summary:

Dream Youth Clinic are the youth-engaged, youth-inspired, adolescent clinics located within the Oakland-based Wellness Centers of Dreamcatcher, Covenant House Youth, Tiny House Village Shelters, and Mobile Clinic street level care, providing comprehensive health services to youth ages 13-25.

Under the supervision of the Medical Director, the Clinical Nurse Practitioner (NP) is responsible for carrying out functions within the NP scope of practice following the principles of primary care and adolescent medicine.

The Nurse Practitioner is an advanced practice nurse that leads with all aspects of patient care, including diagnosis, treatments, and consultations. They will perform both independently and as part of a treatment team. This position will educate patients about preventative care and will prescribe treatments. They will conduct physicals, order tests and serve as a patient's primary healthcare provider. The ideal candidate must have experience serving adolescent, young adult, and/or college populations ideally from underserved communities and communities of color.

Dream Youth Clinic is a Public Health Service Loan Repayment site and this position qualifies for loan repayment hours.

Duties and Responsibilities:

  • Provides services including ongoing primary and preventative care, acute/urgent visits, health assessments, first aid, health education and coaching, referrals to medical and community resources, and patient advocacy.
  • Works as part of our mobile health medical team providing street level medical care to youth.
  • Orders and interprets diagnostic and therapeutic tests relative to the patient's presenting issue(s) as well as age-specific needs.
  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
  • Implements interventions to support the patient to regain or maintain their optimum state of wellbeing.
  • Monitors the effectiveness of interventions and makes appropriate adjustments as indicated.
  • Collaborates with multidisciplinary team members by making appropriate referrals.
  • Facilitates staff, patient and family decision making by providing educational tools.
  • Maintains confidential health records.
  • Actively participates in integrated team care management conferences as needed.
  • Monitors referrals made to radiology, laboratory, specialty care and other services to support the patients' overall care management plan.
  • Provides medical care for of a population of complex, high-need youth patients, including monitoring care plan completion and patient outcomes as needed.
  • Enters and maintains assessments, authorizations, and pertinent clinical information into Electronic Health Record systems.
  • Attends department and organizational meetings as required.

Competencies:

  • Graduate of an accredited school of nursing.
  • Graduate of an accredited Nurse Practitioner Program.
  • Must be a Pediatric Nurse Practitioner (PNP) with minimum 3 years experience providing care to adolescents 13-24 or a Family Nurse Practitioner (FNP) with minimum 1 year experience.
  • Current Advanced Practice Registered Nurse (APRN) Licensure.
  • Board certified or board eligible (expected to obtain within 6 months of employment)
  • Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS); PALS provider card.
  • Strong computer skills, including Word, Excel, and PowerPoint.
  • Proficiency with use of electronic medical records
  • Familiar with telemedicine systems and willingness to provide telemedicine visits.
  • Minimum of 1 years experience in the Nurse Practitioner role preferred.
  • Understanding of, sensitivity to, and respect for the diverse academic, socioeconomic, cultural, disability and ethnic backgrounds of community members.
  • Willingness to work a flexible schedule.
  • Ability to establish and maintain effective working relationships with individuals and groups, both professional and non-professional, co-workers, management personnel, the public and others.
  • Solid organizational skills including attention to detail and multi-tasking.
  • Clear and effective external and internal, verbal and written, communication skills.
  • Enthusiastic about providing mobile clinic street level health care for youth
  • Enthusiastic about to work with youth from diverse backgrounds with complex vulnerabilities ( sex trafficked youth, youth experiencing homelessness, justice and foster system involved youth, youth with substance use disorder, youth with complex trauma, etc.).
  • Enthusiastic about working with colleagues from diverse racial, gender, and cultural backgrounds
