Job Summary
We are seeking a compassionate and experienced
Home Health Physical Therapist
to provide high-quality physical therapy services to adult and geriatric patients in their homes. This role focuses on comfort, functional maintenance, safety, and quality of life rather than aggressive rehabilitation. The Physical Therapist will work collaboratively with an interdisciplinary care team to support patients in remaining safely in their residences as long as possible.

Key Responsibilities

  • Provide physical therapy services according to an individualized plan of care
  • Assess and evaluate patients' functional status, including strength, mobility, gait, range of motion, and transfer abilities
  • Evaluate the home environment for safety and functional needs
  • Develop and direct physical therapy treatment plans focused on comfort, safety, and functional maintenance
  • Recommend and provide adaptive equipment to improve patient independence and function
  • Educate patients and caregivers on therapeutic exercises, mobility techniques, and use of assistive devices
  • Instruct patients and families in the proper use and care of therapeutic and prosthetic devices
  • Monitor patient response to treatment and communicate changes in condition to the physician
  • Initiate physical therapy programs and collaborate with caregivers and interdisciplinary team members

Qualifications

  • Doctorate or Master's degree in Physical Therapy from an accredited program
  • Licensed Physical Therapist in the state of California
  • Minimum 1 year of experience as a Physical Therapist
  • Home health or hospice experience preferred
  • Strong verbal, written, and organizational skills
  • Current CPR certification
  • Understanding of hospice philosophy with a focus on comfort and quality of life
  • Valid driver's license with reliable, insured transportation
