Overview:
Carease Health is looking for an experienced and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to provide high-quality, preventative care in skilled nursing facilities. This hybrid position blends in-person visits with remote work and is fully supported by a dedicated scribe team to streamline your documentation.
Success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Requirements:
Active NP or PA license (in the state of practice)
1+ year of experience in:
Geriatric care
Documentation review
Excellent communication, collaboration, and teamwork skills
