Health Information Specialist

Reporting to the Director of Compliance, Privacy, and Security, the Health Information Specialist organizes and handles medical records and health information data. Ensures that information maintains its quality, accuracy, accessibility, and security in both paper files and Electronic Medical Records (EMR) consistent with medical, administrative, ethical, legal, and regulatory requirements.
Essential Functions

- Protects the security of medical records to ensure that confidentiality is maintained.
- Ensure accuracy and HIPAA compliance, ensuring patient information is sent securely to the correct party.
- Sends required Home Care and Hospice documentation to external facilities as required.
- Always uses appropriate methods to send documentation for each facility and does not deviate from the agency process without management approval. (secure email, fax, etc.)
- Sends required documentation to patients according to regulations and agency policy.
- File/scan and attach all components of the patient's medical record accurately and in a timely manner.
- Process incoming and outgoing physician orders as requested.
- Reprints and resends overdue orders to physician practices as requested.
- Checks for rejected orders, Plans of Care (POC), and Certifications of Terminal Illness (CTI) and follows through with corrections daily.
- Correct orders incorrectly categorized to follow physician signature requirements.
- OASIS/HIS Submissions
- Submits data to 3rd party vendor for Patient Satisfaction Surveys
- Realign episodes within the medical record for changes in payor as necessary.
- Researches returned mail due to an incorrect physician mailing address. Makes corrections in the EMR physician database and/or patient profile and resends orders.
- Process discharges of patient records.
- Produces and distributes reports as requested.
- Communicate effectively with clinical staff to request completion or correction of the medical record and/or orders.
- Assists with adding new physicians and facilities to the EMR physician database.
- Box/label discharged medical records for Record Center as requested.
- Learns other tasks to assist in department coverage
Other Job Functions
- Cover Agency Reception desk and phones as assigned/needed
- Answers all incoming calls, managing a system of multiple lines.

Expected Hours of Work
The team member will be scheduled to work Monday – Friday: 8:30 a.m. – 4:30 p.m. or 8:00 a.m. - 4:00 p.m.
37.5 hours per week.
Requirements:
- Associates degree or High School Diploma/GED with equivalent work experience
- Proficiency in Microsoft applications, with advanced skill using Excel
- Navigating and utilizing Electronic Medical Records desired
- One to two years of healthcare experience preferred
