Responsibilities

  • Provide personal care as outlined in the Hospice Aide Plan of Care.
  • Document all care provided accurately and in compliance with agency policies.
  • Report any deviations or changes in the Plan of Care to the Case Manager or designated staff promptly.
  • Safely utilize and operate durable medical equipment as required.
  • Foster teamwork by understanding the roles of other care team members and actively participating in team and staff conferences.
  • Assist with ambulation, transfers, and range of motion exercises as documented in the Hospice Aide Plan of Care.
  • Offer respite care for the patient's family or caregiver during on-site visits when appropriate.
  • Actively participate in Quality Assurance and Performance Improvement (QAPI) activities and attend Interdisciplinary Group (IDG) meetings as appropriate.

Qualifications

  • Completion of a minimum of 75 hours of training, including at least 16 hours of supervised practical training and 16 hours of classroom instruction.
  • At least one year of experience as a CNA, hospice aide, or home health aide (hospice experience preferred).
  • Must be at least 18 years old.
  • Valid driver's license with access to an insured, reliable automobile in good working condition.
  • Strong verbal and written communication skills.

Job Environment

  • The ability to access patients' homes, which may not be routinely wheelchair accessible, is required.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
  • Physical activities will include walking, sitting, stooping, standing, and performing minimal to maximum lifting and turning of patients.

Job Type: Full-time

Pay: $ $18.00 per hour
