About the Role
The Social Worker plays a critical role in promoting the well-being of residents through person-centered care. As a member of the interdisciplinary team, the Social Worker advocates for residents, addressing their psychosocial needs and ensuring seamless coordination of care. This position involves close interaction with residents, families, and care staff to support transitions, provide education, and improve quality of life.
Key Responsibilities:
- Serves as an advocate for residents in a person-centered care environment, ensuring their dignity, autonomy, and preferences are respected.
- Functions as an integral member of the interdisciplinary care team, identifying and developing individualized care plans that address psychosocial needs.
- Acts as a liaison between residents, families, and facility staff, facilitating communication and providing education and emotional support.
- Conducts comprehensive psychosocial assessments and interviews to gather necessary information to inform care plans.
- Ensures accurate and timely completion of relevant MDS (Minimum Data Set) sections and Care Area Assessments (CAAs) in accordance with state and federal regulations.
- Participates in and leads care plan meetings, ensuring resident-centered goals are incorporated and updated as needed.
- Coordinates discharge planning and assist residents and families with transitional housing transfers, referrals, and community resources.
- Responds to residents' psychosocial concerns, including but not limited to grief, adjustment to illness or facility placement, behavioral issues, and family dynamics.
- Maintains accurate documentation in compliance with regulatory standards and facility policies.
- Performs additional duties as required to meet the needs of residents and support overall facility goals.
Qualifications:
- Bachelor's degree in social work (BSW) required; master's degree in social work (MSW) preferred.
- Licensed Social Worker (LSW) or equivalent state credential as required by state regulations.
- Minimum of 1–2 years' experience in a healthcare or long-term care setting preferred.
- Knowledge of MDS 3.0 and the care planning process is strongly preferred.
- Strong interpersonal, communication, and organizational skills.
- Ability to work collaboratively with a multidisciplinary team and diverse resident population.
